摘 要:目的 本研究探讨单绒双羊双胎妊娠射频减胎术后炎性细胞因子与妊娠结局的相关性。方法 回顾性2018年1月至2020年12月在河南省人民医院产科接受减胎治疗的59例单绒双羊,抽取孕妇终止妊娠前外周静脉血,检测相关感染指标如白细胞计数、C反应蛋白、降钙素原、炎性细胞因子,分析比较某些特异性炎性细胞因子与妊娠结局的相关性。结果 (1)围产结局:59例减胎孕妇中,有16例术后引产或者流产,早产有35例(59.3%,35/59),足月产有8例(13.6%,8/59),保留胎儿存活率为72.8%(43/59);(2)影响因素分析:经减胎术后根据妊娠结局分为早产组、流产组、足月产组,其中白细胞计数、CRP和PCT、IL-10水平三组间分别比较无统计学意义(P>0.05)。早产组和流产组的特异性炎症因子IL-6、IFN-γ和TNF-α水平均高于足月产组,组间比较差异有统计学意义(P<0.05)。结论 减胎术后应定期检测孕妇外周静脉血某些特异性炎性细胞因子水平,可以适当保胎治疗,预防早产、流产,改善新生儿结局。Objective To investigate the correlation between inflammatory cytokines and pregnancy outcome after radiofrequency fetal reduction in monochorionic pregnancies.Methods A retrospective analysis from January 2018 to December 2020 reduction treatment of 59 cases of pregnant women, extraction of peripheral venous blood before termination of pregnancy, pregnant women to detect infection factors, such as white blood cell count, reactive protein, calcitonin, inflammatory cytokines, analysze and compare inflammatory cytokine effects on pregnancy outcome.Results(1)Perinatal outcome: Among the 59 pregnant women with reduced fetus, 16 cases were induced labor or miscarriage after operation, 35 cases were premature(59.3%, 35/59), 8 cases were full-term(13.6%, 8/59), and the survival rate of preserved fetus was 72.8%(43/59).(2)Analyze influencing factors: they were divided into preterm labor group, abortion group and term labor group according to pregnancy outcome. There was no statistical significance in white blood cell count, CRP, PCT and IL-10 levels among the three groups(P>0.05). The levels of specific inflammatory cytokines IL-6, IFN-γ and TNF-α in the preterm and abortion groups were higher than those of the term group, and the differences were statistically significant(P<0.05).Conclusion The level of inflammatory cytokines in peripheral venous blood of pregnant women should be detected regularly after pregnancy reduction, which can prevent premature birth and abortion and improve the outcome of newborn.
