基于“子午流注”理论探讨改良版塌渍包在儿童消化道术后中的临床疗效观察

摘　　要：目的 分析改良版中药塌渍包在儿童消化道术后的临床使用效果。方法 将行消化道手术的90患儿例使用随机数位表法分为对照组、塌渍包组、子午流注塌渍包组。患儿手术后经评估,对照组术后给予抗感染、补液、术后康复等常规治疗;塌渍包组在常规治疗的基础上给予非择时中药塌渍,每日一次,3天为一疗程;子午流注塌渍包组在对照组常规治疗的基础上根据“子午流注”理论择时给予中药塌渍包外敷腹部,每日一次,3天为一疗程。治疗3天后进行效果评价,观察患儿术后疼痛缓解时间、胃肠道功能恢复情况。结果 (1)临床总有效率:子午流注塌渍包组为93.33%(28/30),塌渍包组为80%(24/30),对照组为63.33%(19/30),差异有统计学意义(P<0.05);(2)子午流注塌渍包组患儿36 h内疼痛缓解时间、首次排气排便时间、腹胀缓解程度均显著低于对照组与塌渍包组,差异有统计学意义(P<0.05)。结论 基于“子午流注”理论使用中药塌渍包在儿童消化道术后的治疗中效果显著,治疗后患儿临床症状消失时间缩短,住院周期缩短,患儿及家属依从性和接受度高,具有临床推广应用意义。Objective To analyze the clinical efficacy of modified Chinese medicine collapse package in treating children after digestive tract surgery.Methods Totally 90 children with gastrointestinal tract surgery were randomly divided into control group, collapse package group and midnight-noon ebb-flow collapse package group by random number table method. The children were evaluated after surgery and randomly divided into groups for treatment. The control group was given routine treatment such as infection prevention, fluid rehydration to maintain water, electrolyte, acid-base balance and postoperative rehabilitation after gastrointestinal surgery.In addition to the conventional treatment in the control group, collapse package was given Traditional Chinese medicine collapse package once a day for 3 days as a course of treatment.On the basis of routine treatment in the control group, the patients in collapse package group were given external application of Traditional Chinese medicine tcollapse package to the abdomen according to the theory of "midnight-noon ebb-flow", once a day for 3 days as a course of treatment.The effect was evaluated after 3 days of treatment, and the postoperative pain relief time and gastrointestinal function recovery were observed.Results(1)The total clinical effective rate was 93.33%(28/30) in the midnight-noon ebb-flow group, 80%(24/30) in the collapse package group and 63.33%(19/30) in the control group, the difference was statistically significant(P<0.05).(2) The pain relief time within 36 hours, the time of first exhaust and defecation, and the relief degree of abdominal distension in the midnight-noon ebb-flow group were significantly lower than those of the control group and the midnight-noon ebb-flow group, with statistical significance(P<0.05).Conclusion Based on the theory of "midnight-noon ebb-flow", the use of Chinese medicine collapse package has a significant effect on the postoperative treatment of children’s digestive tract.
