玉泉丸合生脉散对气阴两虚证2型糖尿病临床疗效及心血管主要危险因素的影响

摘　　要：目的研究玉泉丸合生脉散对气阴两虚证2型糖尿病(T2DM)临床疗效及心血管主要危险因素的影响。方法选择2型糖尿病患者86例,按照随机数表法分为对照组与观察组,各43例。对照组采用常规西医治疗,观察组在对照组基础上采用玉泉丸合生脉散治疗,2组均连续治疗4周。比较2组治疗前后中医证候积分,并对比治疗后临床疗效,治疗前后空腹血糖(PBG)、餐后2h血糖(2h PBG)及高血压、高脂血症、超重或肥胖等心血管主要危险因素,统计随访6个月内心血管事件发生率。结果2组治疗后中医证候积分降低,且观察组低于对照组(P<0.05);观察组总有效率(90.70%,39/43)高于对照组(72.09%,31/43)(P<0.05);2组治疗后PBG、2h PBG水平均降低,且观察组低于对照组(P<0.05);血脂、血压、BMI达标率均升高,且观察组高于对照组(P<0.05);观察组心血管事件发生率低于对照组(P<0.05)。结论玉泉丸合生脉散治疗2型糖尿病疗效显著,能够降低中医证候积分及血糖水平,改善心血管主要危险因素,降低心血管事件发生率。Objective To study the effect of Yuquan Pills combined with Shengmai Powders on type 2 diabetes mellitus(T2DM)with the syndrome of deficiency of both qi and yin and its influence on the major cardiovascular risk factors.Methods Eighty-six patients with T2DM in our hospital from September 2020 to June 2021 were randomly divided into a control group and an observation group,with forty-three cases in each group.The control group was treated with conventional western medicine,while the observation group was treated with Yuquan Pills combined with Shengmai Powders on the basis of the control group.The TCM syndrome scores before and 4 weeks after treatment were compared between the two groups,and their curative effects after 4 weeks of treatment were compared.The blood glucose levels[fasting blood glucose(PBG),2h postprandial blood glucose(2h PBG)]and the main cardiovascular risk factors(hypertension,hyperlipidemia,overweight or obesity)were compared before and after treatment.The incidence of cardiovascular events during the 6 months of follow-up was counted.Results After 4 weeks of treatment,the scores of TCM syndrome in the two groups were decreased,and the observation group was lower than the control group(P<0.05).The total effective rate of the observation group(90.70%,39/43)was higher than that of the control group(72.09%,31/43)(P<0.05).After 4 weeks of treatment,the levels of PBG and 2H PBG in the two groups were decreased,and the observation group was lower than the control group(P<0.05).After 4 weeks of treatment,the compliance rates of blood lipid,blood pressure and BMI in the two groups were increased,and the observation group was higher than the control group(P<0.05).The incidence of cardiovascular events in the observation group was 4.65%,lower than 23.26%in the control group(P<0.05).Conclusion Yuquan Pills combined with Shengmai Powders is effective in the treatment of type 2 diabetes,which can reduce the TCM syndrome scores and blood glucose levels,improve the main cardiovascular risk factors and reduce
