311 nm窄谱中波紫外线照射治疗寻常性银屑病的疗效及安全性观察

摘　　要：目的 研究分析寻常性银屑病应用311 nm窄谱中波紫外线照射(NB-UVB)治疗的影响。方法 按随机数字表法,将开封市人民医院2020年12月至2022年6月收治的86例寻常性银屑病患者分为两组,常规组(N=43,常规治疗)与窄谱组(N=43,常规治疗联用NB-UVB治疗),对比两组疗效、皮损面积和严重度指数(PASI)、PASI改善率、细胞因子水平、生活质量评分(DLQI)及并发症,进行研究分析。结果 窄谱组疗效更佳,差异有统计学意义(P<0.05)。治疗前,两组PASI评分、外周血中细胞因子水平、DLQI评分差异无统计学意义(P>0.05);治疗后,窄谱组PASI评分,IFN-γ、IL-2、IL-8水平,DLQI评分低于常规组,IL-4水平高于常规组,差异有统计学意义(P<0.05)。治疗后,窄谱组PASI改善率更高,差异有统计学意义(P<0.05)。窄谱组并发症发生率低于常规组,差异有统计学意义(P<0.05)。结论 寻常性银屑病患者应用NB-UVB治疗,临床症状有所改善,有较高安全性,还可以调节细胞免疫,减少并发症,提高生活质量。Objective To explore the clinical effectiveness and safety of 311 nm narrow-band ultraviolet b rays(NB-UVB)in treating the psoriasis vulgaris.Methods According to random number table,a total of 86 psoriasis vulgaris patients admitted to Kaifeng People's Hospital from December 2020 to June 2022 were assigned to routine group(N=43,routine therapy)and narrow-band group(N=43,routine therapy and 311 nm narrow-band ultraviolet b rays).The clinical effects,Psoriasis Area and Severity Index(PASI),PASI improvement rates,cell factors,the Dermatology Life Quality Index(DLQI)scores and treatment complications were compared between two groups.Results The effect of narrow-band group was better,with statistical significance(P<0.05);There were no significant differences in the PASI score,cytokine level in peripheral blood and DLQI score between the two groups before treatment (P > 0.05). After treatment, the PASI score, IFN- γ, IL-2, and IL-8 levels, and DLQI score in the narrow-spectrum group were lower than those in the routine group, the IL-4 level was higher than that in the routine group, with statistical significances (P < 0.05);After treatment, the improvement rate of PASI in the narrow-spectrum group was higher, with significant difference (P < 0.05);The complication rate in the narrow-band group was lower than that in the routine group, and the difference was statistically significant (P < 0.05). Conclusion The application of NB-UVB in the treatment of patients with psoriasis vulgaris has remarkable effect, it can ameliorate clinical symptoms, with high safety, and it also can regulate cellular immunity, reduced complications, and improved quality of life.
