摘 要:探究痰湿质代谢综合征(metabolic syndrome,MS)的红外表现及其与棕色脂肪组织(brown adipose tissue,BAT)之间的关系,为临床诊断和治疗痰湿质MS提供客观依据。于2021年8月至2022年4月在中国中医科学院广安门医院南区内分泌门诊及病房收集研究对象,其中健康对照组20例、非痰湿质MS组40例和痰湿质MS组40例。采集受试者一般信息,计算体质量指数(BMI),测量腰围(WC)、收缩压(SBP)、舒张压(DBP),检测甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、空腹血糖(FPG)、空腹胰岛素(FINS)、瘦素(LP)、脂联素(ADP)、成纤维细胞生长因子-21(FGF-21)水平。应用红外热像仪采集受试者冷刺激实验前、后锁骨上区域(supraclavicular region,SCR)红外热图,观察3组受试者SCR红外热图变化,比较SCR体表平均温度差异,分析其SCR的BAT的变化情况。结果显示,与健康对照组相比,MS 2组的WC、SBP、DBP、TG和FPG水平显著升高(P<0.01),HDL-C水平显著降低(P<0.01);与非痰湿质MS组相比,痰湿质MS组痰湿体质转化分显著升高(P<0.01)。从红外热图分析,冷刺激前3组间SCR体表平均温度无差异;冷刺激后与健康对照组相比,MS 2组SCR体表平均温度降低(P<0.05)。冷刺激后,3组SCR最高温度及到达速度比较均为健康对照组(3 min)>非痰湿质MS组(4 min)>痰湿质MS组(5 min);健康对照组和非痰湿质MS组表现为SCR热偏离增大,左、右两侧SCR体表平均温度均显著升高(P<0.01),痰湿质MS组SCR热偏离变化不显著;与健康对照组相比,痰湿质MS组左、右两侧SCR温度升高差值降低(P<0.01,P<0.05);与非痰湿质MS组相比,痰湿质MS组左侧SCR温度升高差值降低(P<0.05);3组SCR体表平均温度变化表现为健康对照组>非痰湿质MS组>痰湿质MS组。与健康对照组和非痰湿质MS组相比,痰湿质MS组FINS、BMI、FGF-21水平显著升高(P<0.01,P<0.05),ADP水平显著降低(P<0.01,P<0.05),LP水平显著高于非痰湿质MS组(P<0.01)。通过临床�This study aimed to explore the infrared manifestation and role of brown adipose tissue(BAT) in phlegm-dampness me-tabolic syndrome(MS), and to provide objective basis for clinical diagnosis and treatment of phlegm-dampness MS. Subjects were selected from the department of endocrinology and ward in the South District of Guang′anmen Hospital, China Academy of Chinese Medical Sciences from August 2021 to April 2022, including 20 in healthy control group, 40 in non phlegm-dampness MS group and 40 in phlegm-dampness MS group. General information, height and weight of the subjects were collected and body mass index(BMI) was calculated. Waist circumference(WC), systolic blood pressure(SBP) and diastolic blood pressure(DBP) was measured. Triglyceride(TG), high density lipoprotein cholesterol(HDL-C), fasting blood glucose(FBG), fasting insulin(FINS), leptin(LP), adiponectin(ADP) and fibroblast growth factor-21(FGF-21) were detected. The infrared thermal image of the supraclavicular region(SCR) of the subjects before and after cold stimulation test was collected by infrared thermal imager and the changes of infrared thermal image in the three groups were observed. In addition, the differences in the average body surface temperature of SCR among the three groups were compared, and the changes of BAT in SCR were analyzed. The results showed compared with the conditions in healthy control group, the levels of WC, SBP, DBP, TG and FPG in MS groups were increased(P<0.01), and the HDL-C level was decreased(P<0.01). Compared with non phlegm-dampness MS group, phlegm-dampness MS group had higher conversion score of phlegm dampness physique(P<0.01). According to the infrared heat map, there was no difference in the average body surface temperature of SCR among the three groups before cold stimulation. while after cold stimulation, the average body surface temperature of SCR in MS groups was lower than that in healthy control group(P<0.05).
