基于证素辨证原理的阿尔茨海默病中医目诊研究策略思考

摘　　要：诊疗窗口滞后是目前阿尔茨海默病临床研究过程中面临的关键问题。眼睛作为中枢神经系统唯一可无创检测的视觉区域,其在阿尔茨海默病早期非侵入性诊断中的应用价值受到了广泛的关注。中医学很早就认识到了目在神志疾病诊断中的应用价值,现阶段也已经进行了中医目诊的广泛探索,但仍存在观测指标局限、辨证标准不统一、中医内涵缺乏特异性及多样性、目诊分析缺乏整体思维等方面的问题。因此,笔者在中医整体观念的指导下,以证素辨证思维路径为导向,从观测指标、分类方法、指标中医内涵建立及分析模式4个方面入手,探索阿尔茨海默病现代化中医目诊研究策略,以期为阿尔茨海默病的中医目诊研究提供新的思路与方法,促进阿尔茨海默病临床诊断窗口的前移及治疗策略的优化。Lagging in the diagnosis and treatment window is a key issue in the current Alzheimer’s disease research process.As the only non-invasive area of the central nervous system,the eye has attracted widespread attention for its potential to develop a visualized,quantifiable observation window for Alzheimer’s disease.Traditional Chinese medicine(TCM)has long recognized the application value of the eye in the diagnosis of mental diseases,and has also carried out extensive exploration of eye diagnosis of TCM,but there are still problems such as limited observation indicators,inconsistent differentiation standards,lack of specificity and diversity of TCM connotation,and lack of overall thinking in visual analysis.Therefore,under the guidance of the overall concept,guided by the path of dialectical thinking,this study explores the modern TCM eye diagnosis research strategy of Alzheimer’s disease from four aspects:observation indicators,classification methods,TCM connotation establishment and analysis models,in order to provide new ideas and methods for TCM eye diagnosis research of Alzheimer’s disease,and promote the advancement of the clinical diagnosis window of Alzheimer’s disease and the optimization of treatment strategies.
