基于痛敏化和红外热成像技术探讨抑郁症(肾阳虚证)督脉经穴反应

摘　　要：目的:基于痛敏化和红外热成像技术,总结原发性抑郁症(肾阳虚证)的督脉阳性反应腧穴及其配伍规律,提出温肾通督法治疗原发性抑郁症(肾阳虚证)的可行性。方法:临床招募自愿受试者和门诊患者共146例,分为健康组和观察组[抑郁症(肾阳虚证)患者],每组各73例。应用传统腧穴诊察法和红外热成像仪,对受试者的督脉腧穴压痛诊察和体表温度检测,并进行聚类和关联规则分析。结果:聚类分析:将抑郁症肾阳虚证患者督脉阳性反应腧穴分为两大类,一类是主要阳性反应腧穴,即百会、印堂、腰阳关、命门;另一类是次要反应腧穴,得到4个聚类群,分别是脑户、腰俞、风府;脊中、陶道、至阳;灵台、神道;中枢。关联规则:百会、印堂、腰阳关、命门的关联性最强。结论:百会、印堂、腰阳关、命门是抑郁症(肾阳虚证)督脉主要阳性反应腧穴,提出温肾通督法治疗本病的可行性。Objective:According to the theory of pain sensitization and infrared thermal imaging techniques,the positive reaction acupoint and their compatibility rules of the Du meridian in primary depression(kidney yang deficiency syndrome)were summarized,and the feasibility of the method that‘warming the kidney yang and clearing Du meridian’was proposed to treat primary depression(kidney yang deficiency syndrome).Methods:A total of 146 clinical volunteers and outpatients were recruited and divided into healthy group and observation group,namely healthy people and patients with primary depression(kidney yang deficiency syndrome),with 73 cases in each group,using infrared thermal imaging device and traditional acupoint diagnosis method,the volunteers’body temperature and tenderness examination of Du meridian acupoints were measured and tested.Results:Cluster analysis:The Du meridian positive reaction acupoints of primary depression(kidney yang deficiency syndrome)were divided into two categories,one was the main Du meridian positive reaction points,namely Baihui(DU 20),Yintang(EX-HN 3),Yaoyangguan(DU 3),and Mingmen(DU 4);the other was the secondary reaction acupoints for 4 cluster groups,namely Naohu(DU 17),Yaoshu(DU 2),Fengfu(DU 16);Jizhong(DU 6),Taodao(DU 13),Zhiyang(DU 9);Lingtai(DU 10),Shendao(DU 11);Zhongshu(DU 7).Association rules:Baihui(DU 20),Yintang(EX-HN 3),Yaoyangguan(DU 3),Mingmen(DU 4)were the most relevant.Conclusion:Baihui(DU 20),Yintang(EX-HN 3),Yaoyangguan(DU 3),and Mingmen(DU 4)are the main positive reaction acupoints of the Du meridian of the kidney-yang deficiency depression.The internal relevance between the two puts forward the feasibility of‘warming the kidney yang and clearing Du meridian’method to treat depression of kidney yang deficiency syndrome.
