从火热论探析化疗性口腔溃疡的辨治

摘　　要：基于刘完素“火热论”结合临床实践,探讨化疗性口腔溃疡的辨治思路。将化疗性口腔溃疡分为阳热亢盛、实火上炎证,失于潜藏、虚火上浮证及升降失序、郁火上攻证。治疗时应分虚实、重升降,实火上炎者,多以泻黄散清热凉血,升散解毒;虚火上浮者,多以补中益气汤甘温益气,敦厚中土;郁火上攻者,多以升降散燮理升降,清透郁火而收敛愈溃,为化疗性口腔溃疡的辨证施治提供思路。From the perspective of LIU Wansu's“fire heat theory",we further develop and extend our understanding of the differentiation and treatment of chemotherapeutic oral ulcers by combining with the clinical experience.We divided chemotherapeutic oral ulcers into three types including exuberance of yang heat and excess fire flaming upward pattern,loss of storing and upward floating of deficiency fire pattern,and disorderly ascending and descending and constraint fire upward attack pattern.For treatment,it is recommended to differentiate deficiency from excess,and put focus on ascending and descending.If there is excess fire flaming upward,Xiehuang Powder(泻黄散)is mostly used to clear the heat and cool the blood,and resolve toxins by ascending and dissipating;for those with deficiency fire flaming upward,Buzhong Yiqi Decoction(补中益气汤)as sweet-warm formula is often used to boost qi and thicken the center earth;for those with constraint fire upward attack,Shengjiang Powder(升降散)is usually used to regulate the ascending and descending,clear and vent the constraint fire,as as to astringe and cure the ulcer.All these could provide new ideas and strategies for the differentiation and treatment of chemotherapeutic oral ulcers.
