肾脏囊性占位超声造影及CT增强的表现与Bosniak分级的临床应用研究

摘　　要：目的探讨超声造影和增强CT于肾脏囊性占位Bosniak分级的诊断及鉴别诊断的临床应用价值。方法回顾性分析2013年3月至2022年8月共50例经手术病理明确诊断的肾脏囊性占位患者临床资料,所有患者均于同时段进行超声造影与CT增强检查,并予Bosniak分级。结果手术患者中超声造影分级为Ⅰ级3例(6%),Ⅱ级10例(20%),ⅡF级10例(20%),Ⅲ级14例(28%),Ⅳ级13例(26%),CT增强分级为Ⅰ级3例(6%),Ⅱ级15例(30%),ⅡF级5例(10%),Ⅲ级12例(24%),Ⅳ级15例(30%)。此两种检查方法诊断结果均与手术金标准结果高度一致(Kappa值分别为0.841和0.682)。结论超声造影与CT增强对于肾脏囊性占位有相仿的定性诊断准确率,超声造影具有独立的临床应用优势价值。Objective To explore the clinical application value of contrast-enhanced ultrasound and contrast-enhanced CT with Bosniak classification differentiating the malignant cystic renal mass from the benign one.Methods The clinical data of 50 patients with renal cystic masses diagnosed by surgical pathological results from March 2013 to August 2022 were analyzed retrospectively.All patients underwent contrast-enhanced ultrasound and CT enhancement at the same time with reference of Bosniak classification.Results There were 3 cases(6%)with classⅠ,10 cases(20%)with classⅡ,10 cases(20%)with classⅡF,14 cases(28%)with classⅢ,13 cases(26%)with classⅣby contrast-enhanced ultrasound respectively,and 3 cases(6%)with classⅠ,15 cases(30%)with classⅡ,5 cases(10%)with classⅡF,12 cases(24%)with classⅢ,15 cases(30%)with classⅣby CT enhancement respectively.The diagnostic results of the two methods were highly consistent with the surgical pathological results(Kappa values were 0.841 and 0.682 respectively).Conclusion Contrast-enhanced ultrasound and contrast-enhanced CT have similar clinical application values for cystic renal mass,and contrast-enhanced ultrasound has unique advantage over clinical work.
