摘  要:目的了解重症监护病房(intensive care unit,ICU)环境中耐碳青霉烯类肺炎克雷伯菌(Carbapenem-resistant Klebsiella pneumoniae,CRPK)的分布及同源性,为有效防控院内感染提供依据。方法2022年5月18日对黄石爱康医院ICU环境进行采样,监测CRKP定植情况,对ICU环境中的CRKP菌株及该病房已确诊感染CRKP患者携带病原菌进行耐药基因及同源性分析。结果环境物表中CRKP分离率为12.90%,且与患者感染菌的耐药基因均主要为blaKPC-2型(占比83.33%),分析显示呈高度同源性,相似系数达92%~98%。结论ICU环境物表中CRKP定植比例较高,多为泛耐药或全耐药且具有同源性,提示院内环境开展CRKP主动监测的重要性。Objective To understand the distribution and homology of carbapenem-resistant Klebsiella pneumoniae(CRPK)in intensive care unit(ICU)environment,so as to provide the basis for effective prevention and control of nosocomial infection.Methods The ICU environment of Huangshi Aikang Hospital was sampled on 18th May 2022 to monitor CRKP colonization and to analyze the resistance genes and homology of CRKP strains in the ICU environment and pathogens carried by patients with confirmed CRKP infection in this ward.Results The separation rate of CRKP in the environment was 12.90%,and blaKPC-2 type was the main drug resistance gene in the bacteria infected with patients(83.33%).The homology analysis showed high homology,and similarity coefficient reached 92%-98%.Conclusion The proportion of CRKP colonization in ICU ward environment is high,and most of them are pandrug-resistant or fully drug-resistant with homology,suggesting the importance of active monitoring of CRKP in hospital environment.
