摘  要:目的:对比MRI黑血成像技术与下肢CT静脉造影(CTV)在评估髂静脉压迫综合征中的应用。方法:收集可疑髂静脉压迫综合征患者83例,在下肢CTV检查前后2 d内行MRI黑血成像T1 SPACE检查;运用5分法对比T1 SPACE序列及下肢CTV检查在下腔静脉、髂总静脉、髂外静脉及股静脉的图像质量;对比2种检查方法所得患侧髂总静脉最大受压处前后径、对侧同层面髂总静脉前后径、患侧髂总静脉远端前后径、对侧髂总静脉远端前后径及狭窄率。结果:2种检查方法在下腔静脉、髂总静脉、髂外静脉及股静脉的图像质量评分差异均有统计学意义,T1 SPACE图像质量评分均低于CTV(均P<0.001);2种方法的图像质量评分均≥3分,可满足临床诊断需求。T1 SPACE及下肢CTV测量的患侧髂总静脉最大受压处前后径、对侧同层面髂总静脉前后径、患侧髂总静脉远端前后径、对侧髂总静脉远端前后径差异均有统计学意义,T1 SPACE所测值均较CTV低(均P<0.001)。T1 SPACE及CTV所测髂总静脉狭窄率差异无统计学意义(P>0.05),且2种方法测量的狭窄率一致性较好,95.18%(79/83)的点在一致性区间内。结论:MRI黑血成像技术T1 SPACE序列具有无创、无电离辐射、无需对比剂、无并发症的优势,可清晰显示髂静脉受压情况,在评估髂静脉狭窄程度上与下肢CTV一致性较好,可为临床诊断髂静脉压迫综合征提供影像学参考。Objective:To compare magnetic resonance dark blood imaging technique and lower extremity computed tomography venography(CTV)in the evaluation of iliac vein compression.Methods:83 patients with iliac vein compression syndrome suspected were collected.All patients underwent lower extremity CTV black blood imaging T1 SPACE.The image quality of the inferior vena cava,common iliac vein,external iliac vein and femoral vein was compared using the 5-point method in the two sets of images.The anteroposterior diameters of the ipsilateral iliac vein at maximum compression,of the contralateral common common iliac vein at the same level,of the ipsilateral and the contralateral distal common iliac vein and the stenosis rates were measured on the two sets of images respectively.Results:There were significant differences in the image quality scores of the inferior vena cava,common iliac vein,external iliac vein and femoral vein between the two imaging methods(all P<0.05).The image quality of T1 SPACE was lower than that of CTV,but the image quality score was 3 or above,which could meet the needs of clinical diagnosis.The anteroposterior diameters of the ipsilateral common iliac vein at maximum compression,of the contralateral common iliac vein at the same level,of the ipsilateral and contralateral distal common iliac vein measured by T1 SPACE were lower than those measured by CTV(all P<0.001).The stenosis rate measured by T1 SPACE was not significant difference compared with that measured by CTV(P>0.05),and two groups had good consistency in the measurement of stenosis rate,95.18%(79/83)of the points were within the consistency interval.Conclusions:The dark blood imaging has the advantages of being trauma-free,ionizing radiation-free,contrast-free,and complication-free.It can clearly display the compression of the iliac vein.The images of T1 SPACE and lower extremity CTV have good consistency in evaluating the degree of iliac vein stenosis.T1 SPACE can provide an imaging reference for clinical evaluation of iliac vein compressio
