经后路椎旁肌间隙入路椎弓根钉系统治疗单节段胸腰椎骨折的应用效果

摘　　要：目的:观察经后路椎旁肌间隙入路椎弓根钉系统治疗单节段胸腰椎骨折应用效果。方法:选取2020年6月-2022年7月广东省清远连州市人民医院脊柱关节外科收治的单节段胸腰椎骨折经椎弓根螺钉固定患者60例为观察对象,根据随机数字表法分成对照组与观察组,各30例。观察组采用经后路椎旁肌间隙入路,对照组采用后正中手术入路,比较两组患者围术期指标、胸腰椎功能评分、腰背肌肉疼痛评分。结果:观察组术后椎体前缘高度比高于对照组,手术时间短于对照组,术中出血量与术后住院费用少于对照组,术后并发症发生率低于对照组,差异有统计学意义(P<0.05);术后1周、术后1个月、术后3个月,两组患者胸腰椎功能评分均降低,观察组低于对照组,差异有统计学意义(P<0.05);术后1周、术后1个月、术后3个月,两组患者腰背肌肉疼痛评分均降低,观察组低于对照组,差异有统计学意义(P<0.05)。结论:在椎弓根钉系统治疗单节段胸腰椎骨折治疗中,经后路椎旁肌间隙入路可显著改善椎体前缘高度比,缩短手术时间,减轻患者疼痛,改善患者胸腰椎功能,且并发症较少。Objective:To observe the application effect of posterior paraspinal muscle gap approach pedicle screw system in the treatment of single-level thoracolumbar fracture.Methods:Sixty patients with single-level thoracolumbar fracture treated with pedicle screw fixation in the Department of Spinal Joint Surgery,Qingyuan Lianzhou People's Hospital from June 2020 to July 2022 were selected as the observation objects,and were divided into control group and observation group according to random number table method,with thirty cases in each group.The observation group adopted the posterior paraspinal muscle gap approach,and the control group adopted the posterior median approach.Perioperative indexes,thoracolumbar function scores and lumbar back muscle pain scores were compared between the two groups.Results:The ratio of anterior edge height of vertebral body after surgery in the observation group was higher than that in the control group,and the operation time in the observation group was shorter than that in the control group.The intraoperative blood loss and postoperative hospitalization cost in the observation group were lower than those in the control group,and the incidence of postoperative complications in the observation group was lower than that in the control group,the difference was statistically significant(P<0.05).At 1 week,1 month and 3 months after surgery,the thoracolumbar function scores were decreased in the two groups,and the scores in the observation group were lower than those in the control group,the difference was statistically significant(P<0.05).At 1 week,1 month and 3 months after surgery,the lumbar back muscle pain scores were decreased in the two groups,and the scores in the observation group were lower than those the control group,the difference was statistically significant(P<0.05).Conclusion:In the treatment of single-level thoracolumbar fracture with pedicle screw system,posterior paraspinal muscle gap approach can significantly improve the ratio of anterior edge height of vertebral body,shor
