摘  要:目的探讨游离股前外侧嵌合穿支分叶肌皮瓣修复肢体腔隙性创面的临床疗效。方法2016年6月-2020年12月,收治56例合并骨缺损、骨关节外露、肌肉(腱)缺损或外露的四肢腔隙性创面患者。采用股前外侧嵌合穿支分叶肌皮瓣修复,股外侧肌瓣填塞腔隙,穿支分叶皮瓣修复皮肤软组织创面。供区直接缝合。结果术后56例皮瓣均顺利成活,受区创面与供区切口均一期愈合。患者均获随访,随访时间6~24个月,平均15个月。所有皮瓣颜色、质地良好,股四头肌肌力正常,膝关节活动无影响。结论股前外侧嵌合穿支分叶肌皮瓣可立体、灵活地修复肢体腔隙性创面,是临床修复肢体腔隙性创面的有效方法之一。Objective To investigate the efficacy of the chimeric anterolateral thigh perforator lobulated myocutaneous flap to cure limb multiple lacunar wounds.Methods From June 2016 to December 2020,56 patients with bone defects,or tendon and joint exposure were treated and reconstructed with the chimeric anterolateral thigh perforator leaf flap.All the wounds were reconstructed with the chimeric anterolateral thigh perforator lobulated myocutaneous flap,and the muscle flap was inserted into the deep dead space,with perforator flap for skin defect.The defects on the donor sites were closed directly.Results All the flaps were survival without infection and vascular crisis.The wounds of recipient and donor sites healed at first intention.The patients were followed up 6~24 months(mean,15 months).Good color and texture of flaps were achieved.The muscle strength of quadriceps femoris was normal,and the activity of knee joint was not affected.Conclusion The chimeric anterolateral thigh perforator lobulated myocutaneous flap can freely construct three-dimensional soft tissue defect,It is an effective clinical treatment for limb multiple lacunar wounds.
