摘　　要：目的探讨磁共振成像对胎盘残留个体化处理的应用价值。方法选取41例经手术和(或)病理证实的胎盘残留MRI及超声检查结果,比较MRI、超声检查结果与病理和(或)手术结果的一致性。结果MRI、超声诊断单纯胎盘残留、胎盘残留伴粘连、胎盘残留伴植入、胎盘残留伴穿透植入的诊断准确率分别为73.33%、50.00%、90.00%、100%和66.67%、50.00%、80.00%、50.00%。Kappa检验显示MRI与病理和(或)手术结果一致性较好(Kappa=0.670),优于超声(Kappa=0.509)。结论MRI能较好评价胎盘残留类型,对胎盘穿透性植入诊断较超声更具优势,可为临床个体化治疗提供有效信息。Objective To explore the application value of magnetic resonance imaging in the individualized treatment of placental residue.Methods The MRI and ultrasound results of 41 cases of placental residue confirmed by surgery and/or pathology were retrospectively analyzed.The consistency of MRI and ultrasound results with pathology and/or operation results were compared.Results The diagnostic accuracy of MRI and ultrasound for simple placental residue,placental accreta/placental residue with adhesion,placental increta/placental residue with implantation,and placental percreta/placental residue with penetrating implantation were 73.33%,50.00%,90.00%,100%and 66.67%,50.00%,80.00%,50.00%,respectively.Kappa test showed that MRI had a good consistency with the pathology and/or surgical results(Kappa=0.670),which was better than ultrasound(Kappa=0.509).Conclusion MRI can better evaluate the type of placental residue,and has more advantages in the diagnosis of placental percreta than ultrasound,thereby providing effective information for clinical individualized treatment.
