基于IMB模型的结构化护理规程在儿童少年期精神障碍患者中的应用

摘　　要：目的:探讨基于信息-动机-行为技巧(IMB)模型的结构化护理规程在儿童少年期精神障碍患者中的应用效果。方法:选择2019年3月至2020年3月期间聊城市第四人民医院接诊的300例儿童少年期精神障碍患儿纳入研究,按照随机数表法分为观察组和对照组各150例,对照组给予常规护理,观察组给予基于IMB模型的结构化护理规程,均持续护理8 w。比较护理前、护理后2 w、4 w、8 w后,两组患儿的简明精神病评定量表(BPRS)评分变化;比较护理前及护理8 w后,两组的儿童少年生活质量量表(QLSCA)评分变化;护理8 w后,评估两组患儿的治疗依从性及家属对护理的满意度。结果:观察组护理2 w、4 w、8 w后BPRS量表评分均比对照组低,差异有统计学意义(均P<0.05);护理8 w后,观察组患儿QLSCA评分中,社会心理功能、生理心理健康、生活质量满意度评分均高于对照组,差异有统计学意义(P<0.05);护理8 w后,观察组患儿的治疗依从率、家属对护理的满意度均高于对照组,差异有统计学意义(均P<0.05)。结论:基于IMB模型的结构化护理规程在儿童少年期精神障碍患者中应用效果明显,可有效改善精神状况,提高生活质量及治疗依从性,值得临床推广。Objective:To study the application effect of structured nursing procedures based on information-motive-behavior skill(IMB)model in children and adolescents with mental disorders.Methods:A total of 300 cases of children and adolescents with mental disorders admitted in this hospital from March 2019 to March 2020 were included in the study,according to the random number table method,they were divided into observation group and control group,150 cases in each group.The control group was given routine nursing content,and the observation group was given structured nursing based on IMB model,which lasted for 8 weeks.The scores of Brief Psychiatric Rating Scale(BPRS)were compared before nursing,2 weeks,4 weeks and 8 weeks after nursing;the scores of quality of life scale for children and adolescents(QLSCA)were compared between the two groups before and 8 weeks after nursing;after 8 weeks of nursing,the treatment compliance of the two groups of children and the satisfaction of their families with nursing were evaluated.Results:The BPRS scale score in observation group was lower than the control group after nursing 2,4 and 8 weeks,and the difference was statistically significant(P<0.05).After 8 weeks of nursing,observation group with QLSCA score(social psychological,physiological and psychological health,quality of life satisfaction score)were higher than the control group,the differences were statistically significant(P<0.05).After 8 weeks of nursing,the children treatment compliance rate and family satisfaction with nursing in the observation group were higher than the control group,the differences were statistically significant(P<0.05).Conclusions:The structured nursing procedure based on IMB model have obvious application effect in children and adolescents with mental disorders,which can effectively improve mental status,quality of life and treatment compliance,which is worthy of clinical promotion.
