摘　　要：目的探讨Carlson后外侧手术入路手术对胫骨平台后外侧骨折患者的治疗效果。方法选取2020年6月至2022年6月在郑州市金水区总医院接受手术治疗的97例胫骨平台后外侧骨折患者进行回顾性研究,其中44例患者接受Carlson后外侧手术入路手术(观察组),另外53例患者接受传统后正中入路手术(对照组);对比两组的手术过程指标、术后指标、术后膝关节活动度、膝关节功能。结果两组的手术时间、住院时间、骨折愈合时间、完全负重时间比较差异无统计学意义(P>0.05),观察组的术中出血量少于对照组(P<0.05);术后3、6个月,观察组患者膝关节伸膝最大角度数、屈膝最大角度数高于对照组(P<0.05);术后6个月,观察组患者膝关节Lysholm评分中跛行、支持、绞锁、不稳定、肿胀、上楼、下蹲、疼痛评分均高于对照组(P<0.05)。结论Carlson后外侧手术入路手术在胫骨平台后外侧骨折患者治疗中具有创伤更小、更有利于患者术后膝关节功能恢复的优势。Objective To explore the effect of Carlson posterolateral surgical approach operation on the treatment of posterolateral tibial plateau fractures.Methods A retrospective study was performed in 97 patients with posterolateral tibial plateau fractures treated with surgery in June 2020 to June 2022 Jinshui District General Hospital of Zhengzhou.Of these,44 patients underwent Carlson posterolateral approach operation(observation group)and 53 patients underwent conventional posterior median approach operation(control group).The surgical index,postoperative index,postoperative knee mobility,knee function were compared.Results There was no statistical difference in the operation time,hospitalization time,fracture healing time and total weight-bearing time between the observation group and the control group(P>0.05).The intraoperative blood loss of the observation group was less than that of the control group(P<0.05).At 3 and 6 months after operation,the maximum angle of knee extension knee and the maximum angle of knee flexion in the observation group were higher than those in the control group(P<0.05).At 6 months after operation,the limp,support,lock,instability,swelling,upstairs,squat and pain scores of the knee joint Lysholm score in the observation group were higher than those of the control group(P<0.05).Conclusion The Carlson posterolateral surgical approach operation has the advantage of less trauma and better postoperative knee function recovery in patients with posterior lateral fractures of the tibial plateau.
