摘　　要：目的探究接骨通脉丸口服配合接骨丹外敷治疗膝骨关节炎合并骨髓水肿的临床效果。方法选取2019年6月至2021年6月三门峡市中医院收治的92例膝骨关节炎合并骨髓水肿患者作为研究对象,分为观察组和对照组。对照组接受基础抗炎镇痛药物塞来昔布及硫酸氨基葡萄糖口服治疗,观察组接受接骨通脉丸口服配合接骨丹外敷治疗。对比两组患者疗效和不良反应,并观察两组患者治疗前后视觉模拟评分法(VAS)评分、关节叩击痛程度评分、西安大略和麦克马斯特大学关节炎指数(WOMAC)评分、整体磁共振成像评分系统(WORMS)评分变化。结果治疗后观察组患者的有效率(91.30%)高于对照组(69.57%)(P<0.05)。治疗前两组患者的VAS评分、关节叩击痛程度评分、WOMAC评分、WORMS评分比较差异无统计学意义(P>0.05)。治疗1个月后观察组的VAS评分和叩击痛评分与治疗前和同时点的对照组比较均降低(P<0.05),治疗3个月后两组患者VAS评分和叩击痛评分比较差异无统计学意义(P>0.05)。治疗1个月后观察组的WOMAC评分和WORMS评分与治疗前和同时点的对照组比较均更低(P<0.05),治疗后3个月两组患者WOMAC评分和WORMS评分比较差异无统计学意义(P>0.05)。治疗后观察组与对照组的不良反应发生情况比较差异无统计学意义(P>0.05)。结论采用接骨通脉丸口服配合接骨丹外敷治疗膝骨关节炎合并骨髓水肿患者可以更早减轻患者疼痛感,改善患者的关节功能,降低患者的炎症因子水平,同时不会增加副作用。Objective To explore the clinical study on the treatment of knee osteoarthritis combined with bone edema by oral Jiegu Tongmai Pill and external application of Jiegu Dan.Methods A total of 92 patients with knee osteoarthritis and bone edema admitted to Sanmenxia Hospital of Traditional Chinese Medicine from June 2019 to June 2021 were selected as the study objects,and were divided into observation group and control group.The control group were given basic anti-inflammatory and analgesic drug celecoxib and glucosamine sulfate oral treatment,and the observation group was given Jiegu Tongmai Pill oral combined with Jiegu Dan external.The efficacy and adverse reactions of the two groups were compared,and the changes of visual analogue score(VAS),joint percussion pain degree score,Western Ontario and McMaster University score of knee joint(WOMAC)score,and Western Ontario and McMaster University Osteoarthritis index(WORMS)score of the two groups before and after treatment were observed.Results After treatment,the curative effect of the observation group(91.30%)was higher than that of the control group(69.57%)(P<0.05).Before treatment,there was no statistical difference between the two groups in VAS score,joint percussion pain score,WOMAC score and WORMS score(P>0.05).One month after treatment,the VAS score and percussion pain score of the observation group were lower than those of the control group before treatment and at the same time(P<0.05),but there was no statistical difference between the two groups in VAS score and percussion pain score 3 months after treatment(P>0.05).After treatment,the WOMAC score and WORMS score of the observation group were lower than those of the control group before and at the same time(P<0.05),but there was no statistical difference between the two groups 3 months after treatment(P>0.05).After treatment,there was no statistical difference in adverse reactions between the observation group and the control group(P>0.05).Conclusion Jiegu Tongmai Pill orally combined with Jiegu Dan external
