高强度聚焦超声联合宫腔镜下负压吸宫术治疗剖宫产瘢痕妊娠的临床效果观察

摘　　要：目的观察分析高强度聚焦超声(HIFU)联合宫腔镜下负压吸宫术治疗剖宫产瘢痕妊娠临床效果。方法选取剖宫产瘢痕妊娠患者96例,根据随机数字表法分为观察组与对照组,每组48例。观察组采用HIFU联合宫腔镜下负压吸宫术治疗,对照组采用子宫动脉栓塞联合宫腔镜下负压吸宫术治疗。观察比较两组患者手术相关指标、术后不良反应发生率、术后6个月内计划再次妊娠成功率。结果观察组术中出血量少于对照组,术后阴道流血时间、月经恢复正常时间、住院时间均短于对照组,差异有统计学意义(P<0.05)。两组血清人绒毛膜促性腺激素恢复正常时间比较,差异无统计学意义(P>0.05)。观察组术后不良反应发生率低于对照组,差异有统计学意义(P<0.05)。观察组术后6个月内计划再次妊娠成功率高于对照组,差异有统计学意义(P<0.05)。结论采用HIFU联合宫腔镜下负压吸宫术治疗剖宫产瘢痕妊娠临床疗效显著,术后不良反应少,再次妊娠成功率高,值得临床推广和应用。Objective To observe and analyze the clinical effect of high intensity focused ultrasound(HIFU)combined with hysteroscopic negative pressure aspiration in the treatment of caesarean section cicatricial pregnancy.Methods A total of 96 patients with caesarean section scar pregnancy were selected and divided into observation group and control group according to random number table method,48 cases in each group.The observation group was treated with HIFU combined with hysteroscopic negative pressure aspiration,while the control group was treated with uterine artery embolization combined with hysteroscopic negative pressure aspiration.Operation-related indexes,postoperative adverse reactions,and the success rate of planned pregnancy 6 months after surgery were observed and compared between the two groups.Results The blood loss in the observation group was less than that in the control group,and the postoperative vaginal bleeding time,menstrual recovery time and hospital stay were shorter than that in the control group,the difference was statistically significant(P<0.05).There was no significant difference in the recovery time of human chorionic gonadotropin(β-HCG)between the two groups(P>0.05).The incidence of postoperative adverse reactions in observation group was lower than that in control group,and the difference was statistically significant(P<0.05).The success rate of planned pregnancy in the observation group was higher than that in the control group 6 months after surgery,and the difference was statistically significant(P<0.05).Conclusion HIFU combined with hysteroscopic negative pressure aspiration in the treatment of cicatricial pregnancy by cesarean section has significant clinical efficacy,less postoperative adverse reactions,high success rate of pregnancy,worthy of clinical promotion and application.
