经阴道超声检查在异位妊娠诊断中的应用效果分析

摘　　要：目的探讨采用经阴道超声检查诊断异位妊娠的诊断效能,分析该方式对不同超声征象的检出率。方法纳入100例疑似异位妊娠患者为研究对象,全部患者均接受经腹部超声检查、经阴道超声检查,将追踪确诊结果作为标准,分析两种检查方式的诊断效能,并比较两种检查方式对不同超声征象的检出率。结果以随访确诊结果为标准,100例患者最终确诊84例,经腹部超声检查、经阴道超声检查分别检出其中65、81例。经阴道超声检查对异位妊娠诊断的准确度、特异度、敏感度均高于经腹部超声检查,差异有统计学意义(P<0.05)。经阴道超声检查对胚芽、宫腔内假孕囊、原始心管搏动、附件区团块、盆腔积液的检出率均高于经腹部超声检查,差异有统计学意义(P<0.05)。结论经阴道超声检查诊断异位妊娠准确率较高,且敏感度、特异度均较为理想,能够为医师提供影像学特征图像便于诊断,值得临床推广与应用。Objective To explore the diagnostic efificacy of transvaginal ultrasonography for ectopic pregnancy and analyze the detection efiect of difierent ultrasonic signs.Methods A total of 100 patients suspected of ectopic pregnancy were included in the study.All patients received transabdominal ultrasonography and transvaginal ultrasonography.The diagnostic efficacy of the two methods was analyzed,and the detection rates of different ultrasonic signs were compared.Results According to the diagnosis results of follow-up,84 of the 100 patients were finally diagnosed,and 65 and 81 of them were detected by abdominal ultrasound and transvaginal ultrasound,respectively.The accuracy,specificity and sensitivity of transvaginal ultrasonography in the diagnosis of ectopic pregnancy were higher than those of transabdominal ultrasonography,the difference was statistically significant(P<0.05).The detection rates of transvaginal ultrasonography were higher than those of transabdominal ultrasonography for germ,intraperitoneal pseudogestational sac,original cardiac tube pulsation,adnexal mass and pelvic effusion(P<0.05).Conclusion Transvaginal ultrasonography has high accuracy in the diagnosis of ectopic pregnancy,and its sensitivity and specificity are ideal.It can provide imaging features for doctors to facilitate diagnosis,and is worthy of clinical promotion and application.
