腹针疗法联合脐水消外敷神阙治疗肝癌腹水的临床研究

摘　　要：目的观察腹针疗法联合脐水消外敷神阙治疗肝癌腹水的疗效。方法40例原发性肝癌腹水患者,非随机化分为治疗组和对照组,各20例。对照组给予常规西医对症治疗,治疗组则在对照组基础上给予腹针联合脐水消外敷神阙治疗。比较两组腹水疗效、中医症状积分改善情况、Karnofsky功能状态评分量表(KPS)评分改善情况、不良反应发生情况。结果治疗后,治疗组腹水治疗总有效率为90%,高于对照组的45%,差异有统计学意义(P<0.05)。治疗后,治疗组中医症状积分改善总有效率为90%,高于对照组的35%,差异有统计学意义(P<0.05)。治疗后,治疗组KPS评分改善总有效率为40%,与对照组的15%比较,差异无统计学意义(P>0.05)。两组均未见严重不良反应。结论腹针联合脐水消外敷神阙能短期内改善肝癌腹水患者相关临床症状,减慢腹水生成,但在生活质量方面并未显示出优势。Objective To observe the efficacy of abdominal acupuncture combined with Qishuixiao external application of Shenque point on hepatocellular carcinoma ascites.Methods A total of 40 patients with hepatocellular carcinoma ascites were randomly divided into treatment group and control group,with 20 patients in each group.The control group was treated with conventional western medicine,while the treatment group was treated with abdominal acupuncture and Qishuixiao external application of Shenque point on the basis of the control group.The efficacy of ascites,improvement of traditional Chinese medicine symptom score,improvement of Karnofsky performance status(KPS)score,and occurrence of adverse reactions were compared between the two groups.Results After treatment,the total effective rate of ascites treatment in the treatment group was 90%,which was higher than 45%in the control group,and the difference was statistically significant(P<0.05).After treatment,the total effective rate of improvement of traditional Chinese medicine symptom score was 90%in the treatment group,which was higher than 30%in the control group,and the difference was statistically significant(P<0.05).After treatment,the total effective rate of KPS score improvement in the treatment group was 40%,which was not statistically significant compared with 15%in the control group(P>0.05).No serious adverse reactions were observed in both groups.Conclusion Abdominal acupuncture combined with Qishuixiao external application of Shenque point can improve the clinical symptoms of patients with hepatocellular carcinoma ascites in a short time,and slow down the generation of ascites,but it does not show advantages in terms of quality of life.
