消炎利胆汤对腹腔镜下胆结石手术患者应激反应、胆囊功能及并发症的影响

摘　　要：目的 探讨消炎利胆汤对腹腔镜下胆结石手术患者应激反应、胆囊功能及并发症的影响。方法 选择2018年9月至2021年1月我院收治的88例腹腔镜下胆结石手术患者为研究对象,以随机数字表法将其分为对照组与观察组,各44例。对照组术后给予常规对症治疗,观察组在对照组基础上加用消炎利胆汤。比较两组的应激反应指标、胆囊功能恢复情况、炎性因子水平及并发症发生情况。结果 治疗后3个月,两组的C反应蛋白(CRP)、肾上腺素(E)、皮质醇(Cor)水平均低于治疗前,且观察组低于对照组(P<0.05)。治疗后3个月,两组的胆囊壁厚度、空腹胆囊容积(FV)均小于治疗前,胆囊收缩率大于治疗前,且观察组优于对照组(P<0.05)。治疗后3个月,两组的降钙素原(PCT)、白细胞介素-17(IL-17)、白细胞介素-22(IL-22)水平均低于治疗前,且观察组低于对照组(P<0.05)。观察组的并发症总发生率为4.55%,低于对照组的20.45%(P<0.05)。结论 消炎利胆汤用于腹腔镜下胆结石手术患者术后治疗中可减轻应激反应,促进胆囊功能恢复,也能减少并发症的发生,值得推广。Objective To explore the effects of Xiaoyan Lidan decoction on stress response,gallbladder function and complications of patients undergoing laparoscopic gallstone surgery.Methods A total of 88 patients undergoing laparoscopic gallstone surgery admitted in our hospital from September 2018 to January 2021 were selected as the research objects and divided into control group and observation group by random number table method,with 44 cases in each group.The control group was given conventional symptomatic treatment after surgery,and the observation group was given Xiaoyan Lidan decoction on the basis of the control group.The stress response mdicators,gallbladder function recovery,inflammatory factors and complications were compared between the two groups.Results At 3 months after treatment,the levels of C-reactive protein(CRP),epinephrine(E)and cortisol(Cor)in both groups were lower than those before treatment,and those of the observation group were lower than the control group(P<0.05).At 3 months after treatment,the gallbladder wall thickness and fasting gallbladder volume(FV)of the two groups were smaller than those before treatment,gallbladder contraction rate was greater than that before treatment,and those of the observation group were better than the control group(P<0.05).At 3 months after treatment,the levels of procalcitonin(PCT),interleukin-17(IL-17)and interleukin-22(IL-22)in both groups were lower than those before treatment,and those of the observation group were lower than the control group(P<0.05).The total incidence of complications of the observation group was 4.55%,which was lower than 20.45%of the control group(P<0.05).Conclusion Xiaoyan Lidan decoction can reduce the stress response,promote the recovery of gallbladder function and decrease the occurrence of complications in the postoperative treatment of patients undergoing laparoscopic gallstone surgery,which is worthy of promotion.
