摘 要:目的 探析对糖尿病足患者联合自体骨髓干细胞移植治疗的临床效果。方法 纳入某医院于2019年2月至2021年6月收治的100例确诊糖尿病足患者,遵循随机数字表设计原则将其分组并设为对照组和观察组,每组50例。2组同时给予常规糖尿病足治疗,观察组联合自体骨髓干细胞移植。比较2组患者临床疗效差异及不良反应情况。结果 观察组患者糖尿病足临床有效率(96.0%)显著高于对照组(82.0%),差异有统计学意义(P<0.05)。治疗前2组患者VAS、糖尿病足溃疡面积评价数据比较,差异无统计学意义(P>0.05),治疗后2组患者视觉疼痛模拟评分(VAS)、溃疡面积有所改善,观察组评价优于对照组,差异有统计学意义(P<0.05)。治疗后观察组患者创面肉芽组织中血管内皮生长因子(VEGF)(8 077.65±147.42)IOD、人基质衍生因子-1aSDF-1a(SDF-1a)(5 224.09±125.45)IOD、c-x-c趋化因子受体4型(CXCR4)(14 279.86±289.94)IOD水平显著高于对照组,2组比较差异有统计学意义(P<0.05)。2组患者治疗期间均无出现相关不良反应。结论 为收治糖尿病足患者联合自体骨髓干细胞移植方案效果显著,且安全可靠,值得临床推广。Objective To explore the clinical effect of diabetic foot combined with autologous bone marrow stem cell transplantation.Methods A total of 100 patients with confirmed diabetic foot admitted to a hospital from February 2019 to June 2021 were enrolled and randomly divided into control group(50 cases)and observation group(50 cases).Patients in both groups were given to routine diabetic foot treatment,and those in the observation group were additionally given to autologous bone marrow stem cell transplantation.The clinical efficacy and adverse events were compared between the two groups.Results The clinical effective rate of diabetic foot in observation group was significantly higher than that in control group(96.0%vs 82.0%,P<0.05).There were no significant differences in VAS scores and diabetic foot ulcer area between the two groups before treatment(P>0.05),which were significantly improved in both groups after the treatment,and the improvement was more pronounced in the observation group(P<0.05).After treatment,expression levels of vascular endothelial growth factor(VEGF)(8077.65±147.42 IOD),stromal cell-derived factor 1a(SDF-1a)(5224.09±125.45 IOD),and C-X-C motif chemokine receptor 4(CXCR4)(14279.86±289.94 IOD)in wound granulation tissues of the observation group were significantly higher than those of the control group(all P<0.05).No adverse events in 2 groups during the treatment were reported.Conclusion The treatment of diabetic foot combined with autologous bone marrow stem cell transplantation has a significant therapeutic efficacy and a high safety,which is worthy of clinical promotion.
