摘 要:目的 通过调查石家庄市成年受检者常见检查部位的扫描条件和辐射剂量参数,初步了解石家庄市CT受检者辐射剂量水平。方法 采用分层抽样方法,选取石家庄市22家医院的1 642例典型成年患者为调查对象,并对收集到的扫描条件以及剂量学参数进行分析。结果 石家庄市成年受检者常见检查项目的容积CT剂量指数(CTDIvol)和剂量长度乘积(DLP)值的第75百分位数(P75)分别为:头颅44.35 mGy、630.56 mGy·cm,鼻窦/眼眶33.43 mGy、401.20 mGy·cm,颈部21.51 mGy、475.65 mGy·cm,颈椎18.37 mGy、475.52 mGy·cm,胸部平扫13.73 mGy、492.89 mGy·cm,胸部增强37.60 mGy、1317.36 mGy·cm,腹部平扫22.89 mGy、634.17mGy·cm,腹部增强43.74 mGy、1829.28 mGy·cm,盆腔11.52 mGy、427.21 mGy·cm,腰椎螺旋19.58 mGy、540.68 mGy·cm;三级医院受检者头颅、胸部、腹部DLP的P75低于其他2个级别的医院,且头颅、胸部的有效剂量在不同级别医院中差异有统计学意义(H=11.704、11.025,P<0.05)。结论 石家庄市成年受检者除腹部CTDIvol高于标准诊断参考水平外,其余检查部位均偏低或相差不大,需对腹部CT扫描方案进行优化,在满足图像诊断质量的前提下,实现患者放射防护的最优化。Objective To investigate the scanning parameters and radiation dose values of adults in Shijiazhuang,thus preliminarily elucidating the range of dose in CT examination in this city.Methods A total of 1642 subjects from 22 hospitals in Shijiazhuang were selected by the stratified sampling.Their scanning parameters and radiation dose values were analyzed.Results The 75th percentile(P75)of volume computed tomography dose index(CTDIvol)and the dose length product(DLP)of adults in Shijiazhuang were 44.35mGy and 630.56mGy·cm in the head;33.43mGy and 401.20mGy·cm in the sinus/orbital;21.51mGy and 475.65mGy·cm in the neck;18.37mGy and 475.52mGy·cm in the cervical spine;13.73mGy and 492.89mGy·cm in the plain chest CT scan;37.60mGy and 1317.36mGy·cm in the enhanced chest CT scan;22.89mGy and 634.17mGy·cm in the plain abdominal CT scan;43.74mGy and 1829.28mGy·cm in the enhanced abdominal CT scan;11.52mGy and 427.21mGy·cm in the pelvic cavity;and 19.58mGy and 540.68mGy·cm in the spiral CT of the lumbar spine.P75 of DLPs of the head,chest and abdomen in tertiary hospitals were lower than those of primary and secondary hospitals.Moreover,the effective doses in the head and chest CT examinations were significantly different among primary,secondary and tertiary hospitals(H=11.704,11.025,P<0.05).Conclusion In addition to the pelvic CTDIvol that is higher than the diagnostic reference level,other CT examination parameters in adults of Shijiazhuang are relatively lower or similar.It is necessary to optimize the abdominal CT scanning scheme to optimize the radiation protection and ensure the image quality.
