摘  要:目的评价Sysmex XN-1000血细胞分析仪血小板电阻抗法(PLT-I)通道与血小板荧光法(PLT-F)通道在血小板检测聚集报警中的可信度。方法选取2020年8—11月本院门急诊及住院的329例血小板PLT-I通道有Q-Flag报警聚集的标本,采用PLT-F通道进行检测并制备2张血涂片,以显微镜镜检确认血小板聚集结果为金标准,评价3种检测结果的一致性。结果与显微镜镜检相比,PLT-I与PLT-F通道报警聚集灵敏度和特异性分别为73.33%,49.44%;93.33%,100.00%;Kappa值分别为0.128、0.958。结论Sysmex XN-1000血细胞分析仪血小板PLT-I通道聚集报警假阳性率高,而PLT-F通道与显微镜镜检结果一致性较好,当PLT-I通道报警血小板聚集时可追加PLT-F通道检测辅助辨别血小板聚集,以节省人力,降低复检率,提高实验室内样本周转时间。Objective The reliability of platelet electrical impedance(PLT-I)channel and platelet fluorescence(PLT-F)channel of Sysmex XN-1000 blood cell analyzer in platelet detection aggregation alarms was evaluated.Methods With selection of 329 samples with platelet PLT-I channel with Q-Flag alarm aggregation from the outpatient emergency and inpatient departments of the hospital from August 2020 to November 2020,PLT-F channel was used for detection and 2 blood smears were prepared.The platelet aggregation was confirmed by microscopic examination,and the consistency of the three test results was evaluated with microscopic examination as the gold standard.Results Compared to microscopic examination,the sensitivity and specificity of PLT-I and PLT-F channel alarm aggregation were 73.33%,49.44%;93.33%,100.00%;Kappa values were 0.128 and 0.958,respectively.Conclusions The false positive rate of platelet PLT-I channel aggregation alarm of Sysmex XN-1000 blood cell analyzer is high,and the PLT-F channel has good consistency with the microscopic examination results.When PLT-I channel alarms platelet aggregation,PLT-F channel detection can be added to assist in identifying platelet aggregation,so as to save manpower,reduce the retest rate,and improve the sample turnover time in the laboratory.
