摘 要:目的探讨不同孕周、不同地区、不同程度的双胎妊娠贫血及缺铁性贫血(iron deficiency anemia,IDA)发生率。方法回顾性分析2020年1月1日至2020年12月31日于中国18个省市32家医院(中国妇幼保健协会双胎妊娠专委会双胎贫血学组成员单位)产前检查并分娩的6063例双胎妊娠孕妇的临床资料,统计孕妇年龄,孕期血清铁蛋白(serum ferritin,SF)和血红蛋白(hemoglobin,Hb)数值,分娩孕周,新生儿体重等数据,计算双胎妊娠不同孕周、不同地区、不同程度的贫血及缺铁性贫血的发生率,分析其存在的差异及可能原因。结果孕早、中、晚期贫血发生率分别为8.5%(162/1898)、21.5%(608/2821)、29.3%(1142/3904),差异有统计学意义(χ^(2)=318.560,P<0.005)。孕早、中、晚期缺铁性贫血的发生率分别为1.8%(34/1898)、6.6%(186/2821)、8.7%(339/3904),差异有统计学意义(χ^(2)=100.149,P<0.005)。地区发生率:东北地区21.7%(167/770)、西北地区57.2%(314/549)、西南地区17.7%(25/141)、北部地区29.4%(107/364)、东部地区43.9%(1088/2477)、中南地区30.5%(253/829),差异有统计学意义(χ^(2)=294.350,P<0.005)。结论中国双胎妊娠贫血发生率较高,且随妊娠进展逐渐升高,不同地区贫血发病率存在差异,应结合不同地区情况,加强双胎妊娠孕妇的孕期保健,改善母儿妊娠结局。Objective To discuss the incidence of anemia and iron deficiency anemia in twin pregnancy at different gestational age,in different regions and in different degrees.Methods Analyze the clinical data of 6063 cases of twin pregnancy who have examined and delivered in 32 hospitals of 18 provinces(or cities)of China from January 1,2020 to December 31,2020 retrospectively.Data of maternal age,serum ferritin(SF)and hemoglobin(Hb)during pregnancy,delivery gestational age,and neonatal weight were collected,and the incidence of anemia and iron deficiency anemia at different gestational age,in different regions,and in different degrees in twin pregnancies were calculated,and the differences and possible causes were analyzed.Results The incidence of anemia in the first,second and third trimeste was 8,5%(162/1898),21.5%(608/2821)and 29.3%(1142/3904),respectively.Theχ^(2) value was 318.560,and the P value was<0.005.The incidence of iron deficiency anemia in the first,second and third trimeste was 1.8%(34/1898),6.6%(186/2821)and 8.7%(339/3904)respectively.Theχ^(2) value was 100.149,and the P value was<0.005.Regional incidence:21.7%(167/770)of Northeast China,57.2%(314/549)of Northwest China,17.7%(25/141)of Southwest China,29.4%(107/364)of North China,43.9%(1088/2477)of East China,and 30.5%(253/829)of Central-south China.Theχ^(2) value was 294.350,and the P value was<0.005.Conclusion The incidence of anemia in twin pregnancy is relatively high in China,and gradually increases with the progress of pregnancy.The incidence of anemia is different in different regions,so it is necessary to strengthen the pregnancy care of pregnant women with twin pregnancy based on the situation in different regions,so as to improve the maternal and fetal outcomes.
