摘　　要：目的:观察头部穴域丛刺运动疗法治疗缺血性中风恢复期足下垂的临床疗效,为针灸治疗缺血性中风恢复期足下垂提供新的治疗方案。方法:将60例缺血性中风恢复期足下垂患者按随机数字表法分为观察组和对照组各30例。观察组采用运动针法,予头部穴域丛刺结合运动疗法;对照组采用传统针刺疗法结合运动疗法。两组均在治疗前和治疗后进行下肢Fugl-Meyer(FMA)评分、踝关节主动运动(active range of motion,AROM)评分和步行能力分级(functional ambulation classification,FAC)评价,并根据治疗前后AROM测量值对比评定疗效。结果:与治疗前比较,两组FMA、AROM评分和FAC评级明显提高(P≤0.01),效果显著;观察组AROM评分改善程度优于对照组(P<0.05);FMA评分和FAC评级改善比较,差异无统计学意义(P>0.05)。观察组有效率(90.0%)和对照组有效率(83.3%)比较,差异无统计学意义(P>0.05),但观察组显效率(56.7%)明显高于对照组(46.6%)。结论:头部穴域丛刺运动疗法可明显提高缺血性中风恢复期足下垂患者踝关节主动运动能力,且疗效优于传统针刺疗法。本疗法可提升下肢FMA评分、AROM评分和FAC评级,促进患者康复,提高其生存质量。Objective:To observe the clinical effect of scalp cluster needling exercise therapy in the treatment of foot drop in the convalescent stage of ischemic stroke,and to provide a new treatment plan for acupuncture and moxibustion in the convalescent stage of ischemic stroke.Methods:60 convalescent stage ischemic stroke foot drop patients were divided into observation group and control group with 30 cases in each group according to the random number table method.The observation group adopted exercise acupuncture,combined with scalp cluster needling exercise therapy.While the control group adopted traditional acupuncture combined with exercise therapy.The Fugl-Meyer(FMA)score of the lower limbs,the active range of motion(AROM)score of the ankle joint,and the functional ambulation classification(FAC)were evaluated before and after treatment in both groups,and the curative effect was evaluated according to the comparison of AROM measurement before and after treatment.Results:Compared with before treatment,the FMA,AROM scores,and FAC scores of the two groups were significantly improved(P≤0.01),and the effect was significant.Compared between the two groups,the improvement of the AROM score in the observation group was better than that in the control group(P<0.05).However,there was no significant difference in the improvement of the FMA score and FAC score between the two groups(P>0.05).There was no significant difference between the effective rate of the observation group(90.0%)and the control group(83.3%)(P>0.05),but the significant rate of the observation group(56.7%)was significantly higher than that of the control group(46.6%).Conclusion:Scalp cluster needling exercise therapy can significantly improve the active movement ability of ankle joints in patients with foot drop in the convalescent stage of ischemic stroke,and the curative effect is better than that of traditional acupuncture.It can improve the FMA score,AROM score,and FAC score of the lower limbs,promote the rehabilitation of patients,and improve their qu
