无锡市社区老年人血清糖链抗原125与代谢综合征的相关性研究

摘　　要：目的探讨无锡市社区老年人群血清糖链抗原125(CA125)水平与代谢综合征(MS)的相关性,为将来探索CA125在代谢紊乱性疾病中的应用提供参考。方法于2021年4—11月,在无锡市锡山区和惠山区社区招募60岁及以上常住老年居民为研究对象,进行问卷调查、体格检查和实验室检测。采用R 4.0.2软件进行t检验、Mann-Whitney U检验和χ^(2)检验。采用多因素logistic回归模型分析CA125与MS及其组分的关联。结果共纳入22838名老年人作为研究对象,平均年龄为(69.5±3.3)岁,其中男性10313人(45.2%),>70岁组8944人(39.2%)。MS患者7848例,患病率为34.4%,其中男性患病率(32.4%)低于女性(36.0%),60~70岁组患病率(33.5%)低于>70岁组(35.7%),差异均有统计学意义(P<0.01)。多因素logistic回归分析结果显示,血清CA125水平与中心性肥胖(OR=1.05,95%CI:1.01~1.08)、空腹血糖升高(OR=1.11,95%CI:1.07~1.14)、血压升高(OR=1.09,95%CI:1.05~1.13)、甘油三酯升高(OR=1.20,95%CI:1.16~1.24)和高密度脂蛋白胆固醇降低(OR=1.14,95%CI:1.09~1.19)呈正相关(P<0.05,P<0.01)。血清CA125水平与MS患病风险呈正相关(OR=1.17,95%CI:1.13~1.21,P<0.01);分层分析结果显示,男性(OR=1.14,95%CI:1.08~1.20)和女性(OR=1.20,95%CI:1.15~1.26),60~70岁(OR=1.19,95%CI:1.15~1.26)和>70岁组(OR=1.16,95%CI:1.11~1.21)的血清CA125水平与MS患病风险均呈正相关(P<0.01)。结论无锡市60岁及以上的老年人血清CA125水平与MS患病风险呈正相关。Objective To explore the correlation between serum carbohydrate antigen 125(CA125)and metabolic syndrome(MS)among community elderly in Wuxi city,and provide the reference for utilization of CA125 in metabolic derangement diseases.Methods From April to November 2021,the elderly residents(≥60 years old)from Xishan district and Huishan district of Wuxi city were selected as the subjects,the investigation was conducted with questionnaires,physical examination and laboratory tests.The t test,Mann-Whitney U test andχ^(2)test were used to analyze the data.Multivariate logistic regression model was used to analyze the correlation between CA125 and MS and its components.The used software was R4.0.2 Results A total of 22838elderly were selected as the subjects,the average age was(69.5±3.3)years old.There were 10313 males(45.2%),8944(39.2%)subjects over 70 years old.There were 7848 patients with MS,the morbidity was 34.4%,the male morbidity(32.4%)was significantly lower than the female morbidity(36.0%),the morbidity(33.5%)of 60-70 years old group was significantly lower than that(35.7%)of>70 years old group,P<0.01.Multivariate logistic regression analysis showed that the serum CA125 level was positively correlated with central obesity(OR=1.05,95%CI:1.01-1.08),increased fasting plasma glucose(OR=1.11,95%CI:1.07-1.14),increased blood pressure(OR=1.09,95%CI:1.05-1.13),increased triglyceride(OR=1.20,95%CI:1.16-1.24)and decreased high density lipoprotein-cholesterol(OR=1.14,95%CI:1.09-1.19),P<0.05,P<0.01.The serum CA125 level was positively correlated with the risk of MS(OR=1.17,95%CI:1.13-1.21,P<0.01).The stratification analysis results showed that the serum CA125 level was positively correlated with MS risk in males(OR=1.14,95%CI:1.08-1.20),females(OR=1.20,95%CI:1.15-1.26),subjects(OR=1.19,95%CI:1.15-1.26)with 60-70 years old and subjects(OR=1.16,95%CI:1.11-1.21)over 70 years old(P<0.01).Conclusion The serum CA125 level in residents(≥60 year old)was positively correlated with the risk of MS in Wuxi city.
