摘  要:探讨血清壳多糖酶3样蛋白1(CHI3L1)与老年腹型肥胖2型糖尿病(T2DM)患者早期肾损伤的关系。研究发现患者血清CHI3L1高于健康体检者,合并肾损伤患者血清CHI3L1高于未合并肾损伤患者。CHI3L1诊断患者早期肾损伤的曲线下面积为0.94,灵敏度为96.4%、特异度为89.5%,截断值为2.87 ng/ml。CHI3L1>2.87 ng/ml[2.75(1.25~6.03)]是患者发生早期肾损伤的危险因素。血清CHI3L1水平对老年腹型肥胖2型糖尿病患者早期肾损伤具有较高的预测价值。The purpose of this study was to investigate the relationship between serum chitinase 3-like protein 1(CHI3L1)and early renal injury in elderly patients with type 2 diabetes mellitus(T2DM)and abdominal obesity.Our findings showed that the serum CHI3L1 level was higher in elderly T2DM patients with abdominal obesity than in health chechup subjects,and was also higher in patients who had complicating renal injury than those who did not.CHI3L1 yielded an area under curve of 0.94,a sensitivity of 96.4%,and a specificity of 89.5%with a cut-off value of 2.87 ng/ml in diagnosing early renal injury in these patients.A CHI3L1 level above 2.87 ng/ml[2.75(1.25-6.03)]was considered a risk factor for development of early renal injury in the patients.In summary,serum CHI3L1 level can be highly predictive of early renal injury in elderly T2DM patients with abdominal obesity.
