18β-甘草次酸对失血性休克大鼠急性肺组织损伤及血气变化的影响

摘  要:目的探讨18β-甘草次酸预处理对失血性休克大鼠急性肺组织损伤及血气变化的影响。方法采用股动脉快速放血制作失血性休克大鼠模型。将30只SD雄性大鼠(SPF级)随机分成假手术组、HS模型组、18β-Gly低剂量组(50 mg/kg)、18β-Gly中剂量组(100 mg/kg)及18β-Gly高剂量组(200 mg/kg),每组6只。记录各组大鼠动脉血压及中心静脉压的变化,观察各组大鼠的存活率、肺组织湿/干比(W/D)、肺组织形态学改变及血气变化。结果HS模型组、18β-Gly低剂量组(50 mg/kg)、18β-Gly中剂量组(100 mg/kg)和18β-Gly高剂量组(200 mg/kg)失血性休克3 h后血压和中心静脉压低于假手术组(P<0.05),但18β-Gly低剂量组(50 mg/kg)、18β-Gly中剂量组(100 mg/kg)和18β-Gly高剂量组(200 mg/kg)大鼠血压与中心静脉压变化与HS模型组比较,差异无统计学意义(P>0.05)。18β-Gly低剂量组(50 mg/kg)、18β-Gly中剂量组(100 mg/kg)、18β-Gly高剂量组(200 mg/kg)和假手术组失血性休克3 h存活率高于HS模型组,肺组织W/D低于HS模型组(P<0.05)。HE染色显示,与HS模型组比较,18β-Gly低剂量组(50 mg/kg)、18β-Gly中剂量组(100 mg/kg)及18β-Gly高剂量组(200 mg/kg)各组大鼠肺组织病理损伤程度均减轻,以18β-Gly高剂量组(200 mg/kg)肺组织病理损伤减轻较明显。18β-Gly低剂量组(50 mg/kg)、18β-Gly中剂量组(100 mg/kg)及18β-Gly高剂量组(200 mg/kg)pH、HCO_(3)^(-)、PaO_(2)高于HS模型组,而PaCO_(2)、K^(+)低于HS模型组(P<0.05)。结论18β-甘草次酸能减轻失血性休克大鼠肺组织病理损伤,改善动脉血气变化,有效提高失血性休克大鼠的存活率。Objective To investigate the effects of 18β-glycyrrhetinic acid pretreatment on acute lung tissue injury and blood gas changes in hemorrhagic shock rats.Methods The hemorrhagic shock rat model was established by rapid bloodletting of femoral artery.Thirty male SD rats(SPF)were randomly divided into sham operation group,HS group,18β-Gly low-dose group(50 mg/kg),18β-Gly medium-dose group(100 mg/kg)and 18β-Gly high-dose group(200 mg/kg),with 6 rats in each group.The changes of arterial blood pressure and central venous pressure in each group were recorded,and the survival rate,lung tissue wet/dry ratio(W/D),lung tissue morphology and blood gas changes were observed.Results The blood pressure and central venous pressure of HS model group,18β-Gly low-dose group(50 mg/kg),18β-Gly medium-dose group(100 mg/kg)and 18β-Gly high-dose group(200 mg/kg)were lower than those of sham operation group(P<0.05);However,there was no significant difference in blood pressure and central venous pressure between 18β-Gly low-dose group(50 mg/kg),18β-Gly medium-dose group(100 mg/kg),18β-Gly high-dose group(200 mg/kg)and HS model group(P>0.05).The 3 h survival rate of hemorrhagic shock in 18β-Gly low-dose group(50 mg/kg),18β-Gly medium-dose group(100 mg/kg),18β-Gly high-dose group(200 mg/kg)and sham operation group was higher than that in HS model group,and the W/D of lung tissue was lower than that in HS model group(P<0.05).HE staining showed that compared with the HS model group,the pathological damage of lung tissue in 18β-Gly low-dose group(50 mg/kg),18β-Gly medium-dose group(100 mg/kg)and 18β-Gly high-dose group(200 mg/kg)was reduced,and the pathological damage of lung tissue in 18β-Gly high-dose group(200 mg/kg)was more obvious.The pH,HCO_(3)^(-) and PaO_(2) of 18β-Gly low-dose group(50 mg/kg),18β-Gly medium-dose group(100 mg/kg)and 18β-Gly high-dose group(200 mg/kg)were higher than those of HS model group,while PaCO_(2) and K^(+) were lower than those of HS model group(P<0.05).Conclusion 18β-Gly can alleviate the
