老年内分泌疾病患者的潜在不适当用药及影响因素分析

摘　　要：目的:通过分析老年内分泌疾病患者住院医嘱中的潜在不适当用药(PIM),为促进临床合理用药提供参考。方法:依据《中国老年人潜在不适当用药判断标准2017》(简称中国PIM标准),对老年内分泌疾病患者住院医嘱潜在的不适当用药情况进行分析。采用单因素和多因素分析方法分析潜在不适当用药的影响因素。结果:共有240例病例纳入研究,中位年龄为71(67,77.75)岁,疾病诊断数为8(7,10.75)种,用药数量为10(8,13)个。依据中国PIM标准,193例患者存在与药物相关的PIM,27例患者存在与疾病有关的PIM,合计PIM发生率为80.42%。影响因素分析发现用药数量(OR=1.254,P=0.001)和糖尿病诊断(OR=4.040,P=0.010)是导致PIM发生的影响因素。结论:老年内分泌疾病患者是PIM发生的高危人群,应加强用药监护,减少PIM发生。OBJECTIVE To assess potentially inappropriate medications(PIM) in elderly patients with endocrine diseases, so as to provide a reference for clinical work.METHODS The criteria of potentially inappropriate medications for old adults in China(Chinese PIM criteria) was used as a screening tool to assess potentially inappropriate medication for elderly patients with endocrine diseases.Univariate and multivariate analyses were applied to determine the risk factors of PIM.RESULTS A total of 240 patients were included in the study with a median age of 71(67,77.75) years, 8(7,10.75) diagnosed diseases and 10(8,13) medications.According to the Chinese PIM criteria, there were 193 cases of potentially inappropriate drugs and 27 cases of PIM under morbid state.The incidence of PIM according to the Chinese PIM criteria was 80.42%.The influencing factor analysis indicated that the number of chronic medications(OR=1.254,P=0.001) and the diagnosis of diabetes(OR=4.040,P=0.010) were risk factors for PIM identified by Chinese PIM criteria.CONCLUSION Elderly patients with endocrine diseases are at high risk of PIM,and medication monitoring should be strengthened to reduce the occurrence of PIM.
