集束化护理对危重烧伤患者俯卧位通气效果的影响

摘　　要：目的探讨集束化护理对危重烧伤患者俯卧位通气效果的影响。方法选取80例危重烧伤患者,按照入院治疗时间分为集束化组(2019—2020年,n=40)和对照组(2017—2018年,n=40)。对照组采用常规护理。集束化组在常规护理的基础上,采取包括镇痛镇静评估、气道管理、营养支持、管道维护、皮肤管理在内的集束化护理措施。比较2组患者窒息、坠床、非计划性拔管、误吸、压疮、呼吸机相关性肺炎(VAP)等并发症的发生率和每次俯卧位通气的平均时间。结果集束化组翻身床俯卧位通气期间并发症发生率为32.5%,明显低于对照组的62.5%,差异有统计学意义(P<0.05);集束化组每次俯卧位通气的平均时间为(7.16±1.62)h,明显长于对照组的(6.28±1.77)h,差异有统计学意义(P<0.05)。结论集束化护理可有效降低危重烧伤患者使用翻身床时并发症的发生率,延长俯卧位通气的时间。Objective To explore the effect ofcluster nursing on prone position ventilation in critically burned patients.Methods According to the time of admission,80 critically burned patients were divided into cluster group(2019-2020)and control group(2017-2018),with 40 patients in each group.Both groups received routine care.Additionally,the cluster group was given cluster nursing measures,including analgesia and sedation assessment,airway management,nutritional support,tube maintenance,and skin management.The incidence of complications such as asphyxia,falling from bed,unplanned extubation,aspiration,pressure soresand ventilator-associated pneumonia and the average time of each prone ventilation were compared between the two groups.Results The incidence of complications during prone ventilation in the cluster group(32.5%)was lower than that in the control group(62.5%)(P<0.05).The average time of each prone ventilation in the cluster group((7.16±1.62)h)was longer than that in the control group((6.28±1.77)h)(P<0.05).Conclusion Cluster nursing can effectively reduce the incidence of complications during theuse of rotating bed and prolong the time of prone position ventilationin critically burned patients.
