摘  要:目的:采用体外冲击波穴位刺激作为干预措施治疗兔模型膝骨关节炎(KOA),探讨体外冲击波穴位刺激通过miRNA信号系统对KOA关节软骨的调节机制。方法:将新西兰大白兔30只随机分为正常组和造模组,采用石膏连续固定的方法造模,造模完成后造模组膝骨关节炎兔随机分为模型组,低、中、高能量冲击波穴位刺激组以及普通针刺组(穴位选取血海、梁丘、足三里、内膝眼、外膝眼、阴陵泉、阳陵泉),分别给与相应的治疗后对兔关节面大体情况进行观察及等级评分、ELISA检测血清及关节液中IL-1β和RANTES、RT-PCR检测关节软骨组织miRNA-146a的表达量,对观察及检测的结果进行数据统计,并得出相应的结论。结果:高能量冲击波穴位刺激干预后模型组兔血清及关节液IL-1β、RANTES含量明显降低(P<0.05)。与正常组比较,模型组兔关节软骨组织miRNA-146a的表达量显著降低(P<0.01),经过冲击波穴位刺激或针刺干预后表达上升(P<0.05),其中以高能量冲击波穴位刺激组上升最为显著(P<0.01),较普通针刺组上升更为显著(P<0.05)。结论:冲击波穴位刺激可有效改善KOA模型兔的关节面的形态、降低血清及关节液中IL-1β和RANTES含量;KOA模型兔的关节软骨组织中miRNA-146a表达降低,通过冲击波穴位刺激治疗后可提高KOA模型兔关节软骨miRNA-146a的表达量。Objective:Extracorporeal shock wave acupoint stimulation was used as an intervention to treat rabbit model knee osteoarthritis(KOA),and the regulation mechanism of extracorporeal shock wave acupoint stimulation on KOA articular cartilage through miRNA signaling system was explored. Methods:30 New Zealand white rabbits were randomly divided into normal and molded groups, The mold is made by continuous fixation of casting, The rabbits were randomly divided into model group, Low, medium and high energy shock wave acupoint stimulation group and ordinary acupuncture group(acupoints selected Xuehai, Liangqiu, Zusanli, Inner knee eye, External knee eye, Yinling Spring, Yangling Spring),Observe the general condition of rabbit articular surface and grade score, detect IL-1 and RANTES in serum and joint fluid, and RT-PCR for the expression of miRNA-146a in articular cartilage tissue, Data statistics of the observation and test results, And draw the corresponding conclusions. Results:The rabbit serum, joint fluid IL-1 and RANTES content were significantly reduced after the high-energy shock-wave acupoint stimulation intervention(P<0.05). Compared with the normal group, the expression of miRNA-146a in the model group was significantly decreased(P<0.01),and the expression increased after shock wave acupuncture stimulation or acupuncture intervention(P<0.05),among which the high-energy shock wave acupuncture. Conclusion:Shock wave acupuncture point stimulation can effectively improve the morphology of articular surface in KOA model rabbits, and reduce IL-1 and RANTES content in serum and joint fluid. reduce miRNA-146a expression in articular cartilage tissue of KOA model rabbits, and increase the expression of miRNA-146a in KOA model rabbit articular cartilage after shock wave acupoint stimulation treatment.
