摘　　要：目的 了解河南省三门峡市新报告艾滋病病毒感染者/艾滋病患者(简称“HIV/AIDS”)CD4^(+)T淋巴细胞(简称“CD4”)首次检测情况和影响因素,为提高新报告HIV/AIDS CD4检测及时性,推动快速启动抗病毒治疗工作提供参考依据。方法 收集2011-2020年三门峡市每年新报告HIV/AIDS的历史卡片,以首次CD4检测时间间隔报告卡录入日期≤14 d定义为检测及时,采用χ^(2)检验和二分类logistic回归分析方法对新报告HIV/AIDS的首次CD4检测及时性进行分析。结果 2011-2020年三门峡市新报告的1 040例HIV/AIDS中,942人(90.6%)检测CD4,首次CD4检测及时率为23.6%。多因素logistic回归分析结果显示,医院报告病例的首次CD4检测及时性是疾控中心报告病例的2.595倍(95%CI:1.460~4.612),省市级单位报告病例的首次CD4检测及时性是县区级单位报告病例的1.634倍(95%CI:1.159~2.302)。结论 三门峡市新报告病例首次CD4检测及时率低,疾控中心应该合理安排检测时间,适当增加检测次数,HIV确证阳性之后尽早进行CD4检测,提高CD4检测及时性。Objective To investigate the status of the first CD4^(+)T lymphocytes test among newly reported HIV/AIDS cases and its influencing factors in Sanmenxia city,and provide references for improving the timeliness of CD4 test among newly discovered HIV/AIDS cases and promoting the rapid start of antiviral treatment in Sanmenxia city.Methods Historical reporting cards of all newly reported HIV/AIDS cases in Sanmenxia from 2011 to 2020 were collected.Test timeliness was defined as the interval between the first CD4 test date and report card entry date≤14 days.The Chi-square test and multivariate binary logistic regression analysis were used to analyze the timeliness for the first CD4 lymphocytes test among newly reported HIV/AIDS cases.Results From 2011 to 2020,942 (90.6%) of the 1 040 newly reported HIV/AIDS cases in Sanmenxia were tested for CD4.The proportion of cases with CD4 test timeliness was 23.6%.Multivariate logistic regression analysis showed that the odds of CD4 test timeliness in hospitals was 2.595 times (95%CI:1.460-4.612) as high as that in CDC,and the odds of CD4 test timeliness in provincial and municipal was 1.634 times (95%CI:1.159-2.302) as high as that in county and district.Conclusion The proportion of cases with first CD4 test timeliness among newly reported cases in Sanmenxia city is low;the CDC should reasonably arrange the testing time and appropriately increase the number of tests;once the newly reported cases are confirmed,the CD4 test should be conducted as soon as possible for improving the timeliness of CD4 test.
