摘 要:目的 了解东台市肠出血性大肠埃希菌(EHEC)O157:H7在不同来源标本(腹泻患者、食品、家畜、昆虫)中的分布情况,为EHEC O157:H7的防控提供科学依据。方法 采集东台市四个乡镇腹泻病人及动物的粪便、生熟肉制品和苍蝇进行病原菌分离培养、鉴定、毒力基因检测。结果 2012-2020年共采集各类标本6 129份,检出74株EHEC O157:H7,检出率为1.21%。其中四种动物粪便中共检出71株,牛羊猪鸡分别为35、27、7、2株,腹泻病人共检出3株,检出率分别为4.86%、3.66%、0.96%、0.27%、0.13%,牛羊带菌率最高。各年份检出率差异有统计学意义(χ^(2)=120.013,P<0.01)。经毒力基因检测,stx2^(+)eae A^(+)hly^(+)31株,eae A^(+)hly^(+)41株,另2株不带任何毒力基因,带毒率97.30%。四种毒力基因检出率差异有统计学意义(P<0.01)。结论 肠出血性大肠埃希菌O157:H7在东台市不同宿主动物和腹泻患者中均有检出,牛、羊是东台市肠出血性大肠埃希菌O157:H7的主要宿主动物,应进一步加强对两种动物及粪便的管理。Objective To understand the distribution of Enterohemorrhagic Escherichia coli(EHEC)O157:H7 in the samples of different sources (patients with diarrhea,food,livestock,insects) in Dongtai city,and provide a scientific basis for the prevention and control of EHEC O157:H7 in Jiangsu province.Methods The samples of feces,raw and cooked meat products and flies of diarrhea patients and animals in four townships of Dongtai city were collected for isolation,culture,identification and virulence gene testing of pathogenic bacteria.Results From 2012 to 2020,a total of 6 129 samples of various types were collected,and 74strains of Escherichia coli O157:H7 were detected,and the positive rate was 1.21%.Among them,a total of 71 strains were detected in the feces of four animals,including cattle (35),sheep (27),pigs (7),and chickens (2),and in patients with diarrhea(3),with the detection rates of 4.86%,3.66%,0.96%,0.27%,and 0.13%,respectively,and the bacteria carrying rates in cattle and sheep were the highest.The difference of positivity rates by year was statistically significant (χ^(2)=120.013,P<0.01).The virulence gene testing showed that 31 strains carried stx2^(+)eae A^(+)hly^(+),41 strains carried eae A^(+)hly^(+),and the other 2 strains did not carry any virulence genes,with a toxicity rate of 97.30%.There was a statistically significant difference in detection rates between the four virulence genes (P<0.01).Conclusion EHEC O157:H7 has been detected in different host animals and patients with diarrhea in Dongtai city,and cattle and sheep are the main host animals of EHEC O157:H7 in Dongtai city,and the management of the two animals and feces should be further strengthened.
