摘 要:目的 了解河南省洛阳市孟津区健康人群A群和C群脑膜炎奈瑟菌(Nm)抗体水平。方法 采用分层随机抽样的方法于2021年在洛阳市孟津区选取健康人群进行问卷调查和血清标本采集,采用酶联免疫吸附试验检测血清中A群和C群Nm IgG抗体,分析其抗体阳性率和中位数浓度(MC)。结果 全部630名调查对象的A群、C群Nm抗体总阳性率分别为69.05%、46.67%(χ^(2)=64.71,P<0.01),MC分别为3.42、1.80μg/mL (H=29.15,P<0.001)。不同年龄组A群Nm抗体阳性率在11.67%~84.55%之间(χ^(2)=118.57,P<0.01),18~39岁组最高;C群Nm抗体阳性率在18.33%~78.33%之间(χ^(2)=106.93,P<0.001),5~6岁组最高。末次免疫后1~<2年,A群和C群的Nm抗体水平均降到最低。有、无、不详免疫史人群A群Nm抗体阳性率分别为60.51%、76.47%、77.59%(χ^(2)=21.36,P<0.001),C群分别为67.26%、20.00%、31.77%(χ^(2)=88.82,P<0.001)。结论 洛阳市孟津区健康人群A群和C群Nm抗体水平偏低,建议优化流脑疫苗免疫策略,探索覆盖婴幼儿、青少年和成人的流脑疫苗免疫体系。Objective To explore antibody levels of group A and C Neisseria meningitis (Nm) among a healthy population of Mengjin district,Luoyang city.Methods Serum samples were collected from healthy individuals selected by a stratified random sampling technique in Mengjin district,Luoyang city in 2021.At the same time,a questionnaire-based survey was conducted among the individuals.The serum samples were tested by an enzyme-linked immunosorbent assay (ELISA) to detect IgG antibodies against serogroup A and C Nm;positive rates and median concentrations (MCs) of them were analyzed.Results Among the 630 subjects,the overall positive rate of antibodies against serogroup A and C Nm was 69.05%and 46.67%,respectively (χ^(2)=64.71,P<0.01).The MCs of the antibodies were 3.42μg/mL and 1.80μg/mL (H=29.15,P<0.001),respectively.By age group,positive rates of antibodies against serogroup A ranged from 11.67%to 84.55%(χ^(2)=118.57,P<0.01),with the highest positive rate in the group aged 18-39 years,and those ranged from 18.33%to 78.33%against serogroup C (χ^(2)=106.93,P<0.001),with the highest positive rate in the group aged 5-6 years.After last immunization 1-<2 years,antibody levels of both group A and C Nm reached to a minimum.The positive rate of antibodies against group A Nm was 60.51%for the group having immunization history,76.47%for the group having no immunization history and 77.59%for the group having unknown immunization history(χ^(2)=21.36,P<0.001),and that against group C Nm was 67.26%for the group having immunization history,20.00%for the group having no immunization history and 31.77%for the group having unknown immunization history,respectively (χ^(2)=88.82,P<0.001).Conclusion Antibody levels of group A and C Nm among the healthy population in Mengjin district,Luoyang city were relatively low.It is suggested to optimize the immunization strategy of meningococcal vaccine,and explore the vaccine immunization system covering infants,teenagers and adults.
