诸暨市无痛分娩现状及问题分析

摘　　要：目的探讨诸暨市无痛分娩现状及存在问题。方法选取2021年1月至12月诸暨市所有医院分娩的7240例产妇病案信息,根据是否行无痛分娩分为无痛分娩组(n=2525)和对照组(n=4715)。比较两组产妇产程、产痛、分娩方式、出血量及产后并发症。结果无痛分娩组和对照组产妇在学历、家庭年收入、医疗支付方式及居住地方面比较差异有统计学意义(p<0.05);两组孕妇职业分布方面比较差异无统计学意义(p>0.05)。无痛分娩组产妇活跃期、第二产程、第三产程均短于对照组,产痛I~II级比例高于对照组,III~IV级比例低于对照组(p<0.05)。无痛分娩组产妇产后会阴损伤和尿潴留低于对照组(p<0.05)。结论无痛分娩很大程度上降低了产妇分娩痛,提升了产时保健质量,但无痛分娩在诸暨市医院普及率不高。Objective To explore the present situation and existing problems of painless delivery in Zhuji.Methods The medical records of 7240 parturients who delivered in all hospitals in Zhuji from January to December 2021 were selected and divided into painless delivery group(n=2525)and control group(n=4715)according to whether painless delivery was performed.The labor process,labor pain,delivery mode,blood loss and postpartum complications were compared between the two groups.Results The differences in terms of education background,family annual income,medical payment method and residence area between the painless delivery group and the control group were of statistically significance(p<0.05).The difference of occupational distribution between the two groups was of no statistically significance(p>0.05).The active period,second stage and third stage of labor in painless delivery group were shorter than those in control group.The proportion of grade I to II labor pain in painless delivery group was higher than that in control group,and the proportion of grade III to IV labor pain was lower than that in control group(p<0.05).Perineal injury and urinary retention in painless delivery group were lower than those in control group(p<0.05).Conclusion Painless delivery can greatly reduce the delivery pain and improve the quality of delivery healthcare,but the penetration rate of painless delivery is not high in Zhuji hospitals.
