摘 要:目的探究父代收缩压与子代收缩压的相关性。方法选择开滦研究中具有父代-子代关系并且体检资料完整者作为观察对象,父代与子代的基线数据为第1次参加体检资料。以父代收缩压作为自变量,子代收缩压作为因变量,使用两水平混合线性模型(家庭以及个体水平、非结构协方差矩阵)分析父代收缩压与子代收缩压的相关性,并且根据子代的性别、年龄进行分层分析。结果符合纳入标准的子代共8867例(其中男性6659例,女性2208例)。平均年龄为31.6岁,平均收缩压为(118.4±14.7)mmHg。父代共纳入8415例,平均年龄为56.9岁,平均收缩压为(133.0±20.0)mmHg。在两水平混合线性模型中,校正其他混杂因素后,父代收缩压每增加1 mmHg,子代的收缩压增加0.06 mmHg[估计值(95%CI)为0.06(0.05~0.06),t=8.92,P<0.01],其中女性、男性子代收缩压分别增加0.05、0.07 mmHg;年龄<30、30~<40、≥40岁子代收缩压分别增加0.06、0.05、0.09 mmHg。结论父代收缩压是影响子代收缩压升高的重要因素并且独立于年龄、性别等传统危险因素。Objective To examine the relationship between paternal systolic blood pressure and offspring systolic blood pressure.Methods The subjects who had the father-offspring relationships and completed physical examinations were observed.The baseline data for the fathers and offspring were the first physical examination data.Using paternal systolic blood pressure as the independent variable and offspring systolic blood pressure as the dependent variable,a two-level mixed linear model(family as well as individual level,unstructured covariance matrix)was used to analyze the association of paternal systolic blood pressure with offspring systolic blood pressure and stratified analysis was carried out according to the offspring gender and age.Results A total of 8867 offspring(including 6659 males and 2208 females)who met the inclusion with mean age of 31.6 years were included and their mean systolic blood pressure was(118.4±14.7)mmHg.A total of 8415 fathers were included,with a mean age of 56.9 years and a mean systolic blood pressure of(133.0±20.0)mmHg.In the two-level mixed linear model,after adjusting for other confounding factors,for every 1 mmHg increase in the systolic blood pressure of the fathers,the systolic blood pressure increased by 0.06 mmHg in the offspring[estimated value(95%CI)was 0.06(0.05-0.06),t=8.92,P<0.01],increased by 0.05 and 0.07 mmHg in female and male offspring,respectively,and 0.06,0.05 and 0.09 mmHg in offspring aged<30,30-<40 and≥40 years.Conclusion Paternal systolic blood pressure is an important factor affecting the increase of offspring systolic blood pressure and is independent of age,gender and other traditional risk factors.
