胰腺术后肠内营养合并2型糖尿病患者的血糖管理现状及影响因素分析

摘　　要：目的总结胰腺术后肠内营养合并2型糖尿病患者的血糖管理现状,并分析相关的影响因素。方法选取2021年1月至2022年6月在湖南省某三级甲等医院肝胆外科住院的180例术后肠内营养合并2型糖尿病的患者为研究对象,分析血糖管理的现状及影响因素。结果153例患者血糖控制不达标,不达标率为85%;通过Logistic分析发现,鼻饲速度、输注途径、肠外营养、胰岛素泵是影响胰腺术后肠内营养合并2型糖尿病患者血糖控制的独立危险因素(P<0.05)。结论胰腺术后肠内营养合并2型糖尿病患者血糖控制的影响因素有鼻饲速度、输注途径、肠外营养、胰岛素泵。临床护理人员应严密监测患者的血糖波动,参照血糖值与医生一同制定胰腺术后合并糖尿病患者营养管理方案,提高血糖控制达标率。Objective To analyze the current status of blood glucose management in patients with enteral nutrition combined with type 2 diabetes mellitus(T2DM)after pancreatic surgery,and to analyze the related influencing factors.Methods From January 2021 to June 2022,180 patients with enteral nutrition combined with type 2 diabetes mellitus who were hospitalized in the Department of Hepatobiliary Surgery of a ClassⅢGrade A hospital in Hunan Province were selected,and the current situation and influencing factors of blood glucose management were analyzed.Results 153 patients'blood glucose control was not up to standard,and the failure rate was 85%.Logistic analysis showed that the nasal feeding speed,infusion route,parenteral nutrition and insulin pump were independent risk factors affecting blood glucose management in patients with T2DM after pancreatic surgery(P<0.05).Conclusions Nasal feeding speed,infusion route parenteral nutrition and insulin pump were independent risk factors affecting blood glucose management in patients with T2DM after pancreatic surgery,nurses should closely monitor the fluctuation of patients'blood sugar,and work out the postoperative nutritional management plan for diabetic patients in pancreatic surgery together with doctors according to the blood sugar level,so as to improve the standard rate of blood sugar management.
