摘　　要：目的探讨保脾手术对创伤性脾破裂患者围术期相关指标及免疫功能的影响。方法选取2019年8月至2021年8月我院收治的64例创伤性脾破裂患者,随机分为两组各32例。对照组予以脾脏切除术治疗,观察组予以保脾手术治疗。比较两组的围术期相关指标、免疫功能及并发症发生情况。结果观察组手术时间、下床活动时间及住院时间短于对照组,术中出血量少于对照组(P<0.05)。观察组术后CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)高于对照组,CD8^(+)低于对照组(P<0.05)。观察组并发症发生率低于对照组(P<0.05)。结论保脾手术治疗创伤性脾破裂效果更佳,可减少术中出血量,缩短术后下床活动时间,减轻免疫功能损害,且并发症少。Objective To explore the impact of spleen-preserving surgery on perioperative related indicators and immune function in patients with traumatic splenic rupture.Methods 64 patients with traumatic splenic rupture admitted to our hospital from August 2019 to August 2021 were selected and randomly divided into two groups,with 32 cases in each group.The control group was given splenectomy,and the observation group was given spleen-preserving surgery.The perioperative related indicators,immune function and complications were compared between the two groups.Results The surgical time,ambulation time and hospitalization time in the observation group were shorter than those in the control group,and the intraoperative blood loss was less than that in the control group(P<0.05).After surgery,the CD3^(+),CD4^(+) and CD4^(+)/CD8^(+)of the observation group were higher than those of the control group,and the CD8^(+)was lower than that of the control group(P<0.05).The incidence of complications in the observation group was lower than that in the control group(P<0.05).Conclusions Spleen-preserving surgery has better effect in the treatment of traumatic splenic rupture,which can reduce the intraoperative blood loss,shorten the postoperative ambulation time,and reduce the damage of immune function,with fewer complications.
