颈脊神经后支松懈联合药物阻滞治疗颈神经后支源性慢性颈肩痛的效果观察

摘　　要：目的观察颈脊神经后支松懈联合药物阻滞治疗颈神经后支源性慢性颈肩痛的临床效果。方法选取2019年5月至2021年5月我院收治的120例颈神经后支源性慢性颈肩痛患者,随机分为对照组(n=60)和观察组(n=60)。对照组采用药物阻滞治疗,观察组在对照组基础上采用颈脊神经后支松懈治疗,比较两组患者的视觉模拟量表(VAS)评分、颈肩肿胀/功能障碍受限评分、生活质量(SF-36)评分及治疗期间不良反应发生率。结果治疗后3 d、7 d、30 d,观察组VAS评分均低于对照组(P<0.05)。治疗后30 d,观察组颈肩肿胀/功能障碍受限评分低于对照组,SF-36评分高于对照组(P<0.05)。治疗期间,两组的不良反应发生率(15.00%vs.23.33%)比较差异无统计学意义(P>0.05)。结论颈脊神经后支松懈联合药物阻滞能够有效缓解颈神经后支源性慢性颈肩痛患者的颈肩疼痛、肿胀及功能受限状况,改善患者的生活质量,且安全性较高。Objective To observe the clinical effect of cervical spinal nerve posterior-branch relaxation combined with drug block in the treatment of cervical dorsal rami chronic neck-shoulder pain.Methods 120 patients with cervical dorsal rami chronic neck-shoulder pain admitted to our hospital from May 2019 to May 2021 were selected and randomly divided into control group(n=60)and observation group(n=60).The control group was treated with routine drug block,while the observation group was treated with cervical spinal nerve posterior-branch relaxation on the basis of the control group.The visual analogue(VAS)score,neck-shoulder swelling/dysfunction limitation score,quality of life(SF-36)score after treatment and adverse reaction rate during treatment period were compared between the two groups.Results 3 d,7 d and 30 d after treatment,the VAS scores in the observation group were lower than those in the control group(P<0.05).At 30 d after treatment,the neck and shoulder swelling/dysfunction limitation scores in the observation group were lower than those in the control group,and the SF-36 score was higher than that in the control group(P<0.05).During the treatment,no statistical difference was found in the incidence of adverse reactions(15.00%vs.23.33%)between the two groups(P>0.05).Conclusions Cervical spinal nerve posterior-branch relaxation combined with drug block can effectively relieve neck-shoulder pain,swelling and functional limitation in patients with cervical dorsal rami chronic neck-shoulder pain,and improve the quality of life with higher safety.
