超声造影时间-强度曲线分析对宫腔占位病变良恶性的诊断价值

摘　　要：目的 探讨超声造影时间-强度曲线分析对宫腔占位病变良恶性的诊断价值。方法 选取2020年1月至2021年12月在丽水市人民医院行超声检查发现的100例宫腔占位病变患者为研究对象,根据宫腔镜术后病理诊断结果分为恶性组48例(其中子宫内膜癌45例,子宫内膜间质肉瘤3例)、良性组52例(均为子宫内膜息肉)。比较两组患者超声造影时间-强度曲线参数,同时分析这些参数对宫腔占位病变良恶性的预测效能。结果 与良性组比较,恶性组对比剂达峰时间较短(P<0.05),峰值强度、曲线上升支斜率均较高(均P<0.05);而两组患者对比剂到达时间比较,差异无统计学意义(P>0.05)。对比剂到达时间、对比剂达峰时间预测宫腔占位病变良恶性的AUC、灵敏度、特异度分别为0.623、0.562、1.000和0.562、0.292、0.923,差异均无统计学意义(均P>0.05);峰值强度、曲线上升支斜率预测宫腔占位病变良恶性的AUC、灵敏度、特异度分别为0.666、0.646、0.712和0.812、0.875、0.673,差异均有统计学意义(均P<0.05)。结论 超声造影时间-强度曲线分析对宫腔占位病变良恶性的鉴别具有一定的诊断价值。Objective To investigate the application of contrast-enhanced ultrasound time-intensity curve analysis in differential diagnosis of benign and malignant uterine space occupying lesions.Methods One hundred patients with uterine space occupying lesions detected by ultrasound examination in Lishui People's Hospital from January 2020 to December 2021 were enrolled.Based on pathological diagnosis results,there were 48 cases of malignant tumor and 52 cases of benign lesions.The parameters of time-intensity curve analysis of contrast-enhanced ultrasound were compared between the two groups,and the predictive factors for benign and malignant uterine space occupying lesions were analyzed by multivariate logistic regression,and their predictive efficacy was analyzed by ROC curve.Results Compared with the benign group,the peak time of contrast agent in the malignant group was shorter,while the peak intensity and the slope of ascending curve were higher(all P<0.05).There was no significant difference in the arrival time of contrast agent between 2 groups(P>0.05).The area of ROC curve(AUC),sensitivity,and specificity of contrast arrival time and contrast peak time for predicting uterine occupying lesions were 0.623,0.562,1.000,0.562,0.292,0.923(all P>0.05),and the AUC,sensitivity,and specificity of peak intensity and slope of the rising curve for predicting uterine occupying lesions were 0.666,0.646,0.712 and 0.812,0.875,0.673(all P<0.05).Conclusion The time-intensity curve analysis of contrast-enhanced ultrasound has certain diagnostic value in the differentiation of benign and malignant uterine space occupying lesions.
