不同剂量低分子肝素钙治疗不稳定型心绞痛的临床疗效研究

摘　　要：目的 探究不同剂量低分子肝素钙治疗不稳定型心绞痛的临床疗效。方法 86例不稳定型心绞痛患者,以随机信封法分为参照组及研究组,各43例。两组均予以常规治疗,参照组在常规治疗基础上予以常规剂量低分子肝素钙治疗,研究组在常规治疗基础上予以大剂量低分子肝素钙治疗。对比两组患者心电图检查结果、随访情况、不良反应发生情况。结果 治疗后,研究组患者T波倒置导联数目(2.08±0.47)个少于参照组的(3.92±0.86)个,T波倒置深度(1.51±0.34)mm、ST段下移深度(1.14±0.28)mm均小于参照组的(2.89±0.53)、(2.51±0.47)mm,差异有统计学意义(P<0.05)。随访6个月,研究组患者复发率、Ⅲ级心绞痛发生率分别为6.98%、0,参照组患者复发率、Ⅲ级心绞痛发生率分别为23.26%、13.95%。研究组患者复发率、Ⅲ级心绞痛发生率均低于参照组,差异有统计学意义(P<0.05)。两组不良反应发生率对比,差异无统计学意义(P>0.05)。且两组均为轻度不良反应,在停止用药后自行改善、消失。结论 于不稳定型心绞痛治疗中,应用大剂量低分子肝素钙治疗的近期效果及远期效果均更好,而且不会增加不良反应。Objective To investigate the clinical efficacy of different doses of low molecular heparin calcium in the treatment of unstable angina.Methods A total of 86 patients with unstable angina were divided into the reference group and the research group by the randomized envelope method,with 43 cases in each group.Both groups were treated conventionally.On this basis,the reference group was treated with conventional dose of low molecular heparin calcium,and the research group was treated with high dose of low molecular heparin calcium.The electrocardiogram results,follow-up,and occurrence of adverse reactions of the two groups were compared.Results After treatment,the number of leads with T-wave inversion(2.08±0.47)leads in the research group was less than(3.92±0.86)leads in the reference group;the depth of T-wave inversion(1.51±0.34)mm and depth of ST-segment depression(1.14±0.28)mm in the research group were less than(2.89±0.53)and(2.51±0.47)mm in the reference group;the differences were statistically significant(P<0.05).At 6-month follow-up,the recurrence rate and the incidence of gradeⅢangina were 6.98%and 0 in the study group,and the recurrence rate and the incidence of gradeⅢangina were 23.26%and 13.95%in the reference group.The recurrence rate and the incidence of gradeⅢangina in the research group were lower than those in the reference group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Both groups had mild adverse reactions,which improved and disappeared on their own after discontinuation of medication.Conclusion In the treatment of unstable angina,the short-term effect and long-term effects of low molecular heparin calcium is better,and there is no adverse reactions.
