摘 要:目的 探讨前白蛋白对新冠病毒感染者的焦虑、超敏C反应蛋白(hs-CRP)和淋巴细胞的影响及中介作用,为预防及改善新冠肺炎愈后提供依据。方法 采用医院焦虑抑郁量表(Hospital Anxiety and Depression Scale,HADS)测量212例新冠病毒感染者的焦虑情况,血液指标如前白蛋白、hs-CRP含量和淋巴细胞数量等由医院检验科统一检测和提供。结果 本次共纳入212例,年龄19~59岁,其中男139例,女73例。年龄(34.67±9.97)岁,住院时长2~27 d。新型冠状病毒感染108例(50.9%),新型冠状病毒肺炎(轻型)71例(33.5%),新型冠状病毒肺炎(普通型)33例(15.6%)。血清前白蛋白与焦虑程度、hs-CRP水平呈负相关(P <0.001),与淋巴细胞数量呈正相关(P <0.001);焦虑与淋巴细胞数量呈负相关(P <0.01);hs-CRP与淋巴细胞数量呈负相关(P <0.001)。中介效应由两条路径组成:前白蛋白→焦虑→淋巴细胞的间接效应1,前白蛋白→hs-CRP→淋巴细胞的间接效应2。研究结果显示,前白蛋白可以通过焦虑、hs-CRP的中介作用对淋巴细胞产生影响(P <0.05)。结论 焦虑、hs-CRP是新冠病毒感染者前白蛋白对淋巴细胞影响的重要中介机制。Objective To investigate the effects of prealbumin on anxiety,hypersensitive C-reactive protein and lymphocytes in patients with COVID-19,and to provide reference for preventing COVID-19 and improving its recovery.Methods The anxiety of 212 patients with COVID-19 was measured by Hospital Anxiety and Depression Scale(HADS),and serum prealbumin,high-sensitivity C-reactive protein and lymphocyte cells were detected by the hospital laboratory.Results A total of 212 people,aged 19-59 years,were enrolled in this study,including 139 men,with an average age of 34.67±9.97 years.The length of hospitalization was 2~27 days.One hundred and eight people(50.9%)were infected with COVID-19,71 of which(33.5%)was with novel coronavirus pneumonia(mild),and 33 people(15.6%)with novel coronavirus pneumonia(common).Serum prealbumin was negatively correlated with anxiety(P<0.001)and the level of high-sensitivity C-reactive protein,and positively with the number of lymphocytes(P<0.001).Anxiety was negatively correlated with lymphocytes(P<0.01),and hypersensitive C-reactive protein was negatively correlated with lymphocytes as well(P<0.001).The mediating effect consisted of two paths:the indirect effect 1 of prealbumin→anxiety→lymphocytes and the indirect effect 2 of prealbumin→hs-CRP→lymphocytes.The results showed that prealbumin could affect lymphocytes through the mediation of anxiety and hs-CRP(P<0.05).Conclusion Prealbumin exerts influence on lymphocytes through the importantly mediating role of anxiety and hypersensitive C-reactive protein in patients with COVID-19 infection.
