摘　　要：目的探讨以三甲医院为主导的康复管理体系对脑卒中偏瘫患者肢体功能恢复的影响。方法回顾性分析2020年12月至2021年9月就诊于河北大学附属医院进行康复治疗的脑卒中偏瘫患者,共90例。所选择病例均先在三甲医院进行康复治疗4周,4周后自行转诊至下级医院进行康复治疗的归为对照组,进入以三甲医院为首的康复管理体系进行康复治疗的归为观察组,2组病例康复治疗周期均为12周。分别在治疗前、治疗4周、治疗8周、治疗12周,使用简式Fugl-Meyer评估量表(Fugl-Meyer Assessment Scale,FMA)进行四肢运动能力评定,使用改良的Barthel指数(Modified Barthel Index,MBI)进行日常生活能力评定。结果不同时间、组别与时间交互作用比较,差异均有统计学意义(P<0.05)。观察组和对照组的FMA评分及MBI评分均随治疗时间延长逐渐增高,且2组间评分差值也随治疗时间延长逐渐增大。结论以三甲医院为首的康复管理系统的施行,能够改善脑卒中偏瘫患者的运动功能和日常生活能力,该体系可行、有效。Objective To explore the effect of rehabilitation management system headed by the 3A hospitals on stroke patients.Methods From December 2020 to September 2021,a total of 90 stroke hemiplegia patients were selected as the research subjects and divided into two groups.The patients of two groups were treated in tertiary hospital,secondary hospital and community rehabilitation for 4 weeks.The cases of the control group were transferred to the lower level hospital by themselves,and the patients of the experimental group entered the rehabilitation management system headed by the 3A hospital to get treatment.Before treatment and 4 weeks after treatment,8 weeks after treatment and 12 weeks after treatment,motor functions were evaluated by Fugl-Meyer motor function score(Fugl-Meyer Assessment Scale,FMA),and activities of daily living were evaluated by modifiedBarthel Index(Modified Barthel Index,MBI).Results After 4 weeks of treatment,there was no significant difference in FMA score and MBI score between the two groups(P>0.05).After 8 and 12 weeks of treatment,the FMA score and MBI score of the experimental group were higher than those of the control group,and the difference was statistically significant(P<0.05).Conclusion The rehabilitation management system headed by the top three hospitals can improve the motor functions and activities of daily living of stroke patients,which is feasible and effective.
