摘 要:目的 比较两种治疗方法防治屈指肌腱缝合术后肌腱粘连的临床疗效。方法 选取在2017年6月~2020年6月期间于安徽省体育科研所康复中心就诊并康复的60例屈指肌腱断裂患者,将患者分为治疗组(大黄素超声离子导入治疗)和对照组(单纯超声导入治疗),分别采用视觉模拟评分法(VAS)、临床肌腱修复疗效标准和健患指屈指力量评定治疗前后的临床疗效。结果 随访60例患者2~12周(平均7周),患者切口全部Ⅰ期愈合,治疗前后患指VAS疼痛评分与对照组(单纯超声导入治疗)2组比较,具有统计学意义;7周后2组患者手功能TAM评分比较,治疗组显效67%(20/30),有效30%(9/30),无效10%(1/30),治疗总有效率97%;对照组显效23%(7/30),有效60%(18/30),无效17%(5/30),治疗总有效率83%。弹性拉伸架装置测试下屈指力量结果显示,治疗组患侧屈指肌力较对照组明显提高,差异具有统计学意义。结论 大黄素经超声离子导入预防治疗屈指肌腱粘连临床疗效可靠,不良反应少,值得临床推广应用。Objective To comare the clinical efficacy of two methods in the prevention and treatment of tendon adhesion after flexor tendon suture.Methods Sixty patients of flexor tendon nature treated in our rehabilitation center from June 2017 to June 2020 were selected and divided into treatment group(emodin ultrasound treatment)and control group(ultrasound guided therapy).The clinical efficacy before and after treatment was evaluated by visual analogue scale(VAS),clinical tendon repair efficacy standard and finger flexion strength of patients.Results All the 60 patients were followed up for 2〜12 weeks(average 7 weeks),and incisions in all patients healed in the first stage.VAS scores of fingers were significantly decreased after treatment compared with the control group(only ultrasound guided therapy),with statistical significance(P<O.05).After 7 weeks,the TAM scores of hand fiinction in the two groups were conpared.In the treatment group,67%(20/30)of treatment were markedly efiective,30%(9/30)were efifective,10%(1/30)were ineflfective,the total eflfective rate was 97%.In the control groups,23%(7/30)and 60%(18/30)of treatment were effective,17%(5/30)were ineffective,and the total efiective rate was 83%.In paiticulai;the flexor strength of the treatment groups was significantly higher than that of the control groups,and the difference was statistically significant Conclusion Ultrasound guided theroy combined with emodin in the prevention and treatment of flexor tendon adhesion has reliable clinical efficacy and less adverse reactions,which is worthy of clinical application.
