超声在Ⅰ型神经纤维瘤病中的诊断价值

摘　　要：目的探讨超声在Ⅰ型神经纤维瘤病中的诊断价值。资料与方法回顾性分析2011年10月—2021年8月郑州大学第一附属医院经病理证实的75例Ⅰ型神经纤维瘤病的超声表现,观察病变部位、数目、形态、边界、内部回声、血流信号以及与周边的毗邻关系。结果Ⅰ型神经纤维瘤病的超声表现呈现多样化和多变性,超声声像图分为结节型、弥漫型、丛状型、混合型,其中多发结节型22例,超声表现为椭圆形或类圆形,边界尚清晰,大多数表现为实性低回声结节;丛状型12例,超声表现为受累神经干扭曲扩张,呈串珠样排列;弥漫型32例,超声表现为皮肤层和皮下脂肪层不均质增厚,边界不清,形态不规则的片状高回声区,其间见条索样低回声;混合型9例,表现为弥漫型和结节型共存,超声图像兼有此两型超声表现。结论超声能够显示Ⅰ型神经纤维瘤病的典型声像图特征,为临床诊疗提供依据。Purpose To investigate the diagnostic value of ultrasound in patient with typeⅠneurofibromatosis.Materials and Methods The ultrasonographic findings of 75 patients pathologically confirmed typeⅠneurofibromatosis in the First Affiliated Hospital of Zhengzhou University from October 2011 to August 2021 were retrospectively analyzed.The location,number,morphology,boundary,internal echo,blood flow signal and the neighborhood relationship between the lesion and the surrounding area were observed.Results The ultrasonic manifestations of typeⅠneurofibromatosis were diversified and varied,and the ultrasonography was divided into four types:nodular type,diffuse type,plexus type and mixed type.Among them,22 cases were of multiple nodules,and the ultrasonic manifestations were oval or almost round with clear boundary,and most of them were solid hypoechoic nodules.There were 12 cases with plexus type,the ultrasonography showed that the involved nerve trunk was twisted and dilated,presenting beaded arrangement.There were 32 cases with diffuse type,the ultrasonic manifestations were uneven thickening of the skin layer and subcutaneous fat layer,with unclear boundary and irregular shape of patchy hyperechoic areas,in which there were cable-like hypoechoic areas.There were 9 cases with mixed type,presenting diffuse type and nodular type co-existed,and ultrasonic images showed both types of ultrasonic features.Conclusion The sonographic characteristics of typeⅠneurofibromatosis were displayed via ultrasound,providing the basis for clinical diagnosis and treatment.
