摘 要:目的:探讨阶梯式健康教育结合一体化管理在冠心病合并心力衰竭患者中的实践作用。方法:选取2018年3月至2021年4月我院收治的80例冠心病合并心力衰竭患者,采用随机数字表法将其随机分为观察组与对照组,各40例。对照组实施常规管理,观察组实施阶梯式健康教育结合一体化管理。对比管理前后冠心病自我管理行为量表(CSMS)及焦虑抑郁评分,并对比管理前后6 min步行试验及生活质量。结果:2组管理后CSMS量表中不良嗜好管理、症状管理、情绪管理、急救管理、疾病知识管理、一般生活管理、治疗依从性各维度评分以及总得分均高于管理前(P<0.05),观察组管理后各维度评分及总得分均高于对照组(P<0.05);2组管理后焦虑自评量表(SAS)、抑郁自评量表(SDS)、明尼苏达心力衰竭生活质量调查表(LiHFe)评分均低于管理前,观察组管理后SAS、SDS、LiHFe评分均低于对照组(P<0.01);2组患者管理后6 min步行距离均明显长于管理前(P<0.01),观察组管理后长于对照组(P<0.05)。结论:对冠心病合并心力衰竭患者采用阶梯式健康教育结合一体化管理,可明显提高患者自我管理行为,减轻其负性情绪,并可促进心功能改善及提高生活质量。Objective:To explore the practice role of stepped health education combined with integrated management in patients with coronary heart disease complicated with heart failure.Methods:A total of 80 patients with coronary heart disease complicated with heart failure treated in our hospital from Mar 2018 to Apr 2021 were selected and randomly divided into the observation group and the control group according to random number table grouping method,with 40 cases in each group.The control group received routine management,while the observation group received stepped health education combined with integrated management.The scores of coronary heart disease self-management scale(CSMS)and anxiety and depression were compared before and after management,and the 6-minute walking test and quality of life before and after management were also compared.Results:The scores of bad hobby management,symptom management,emotion management,first aid management,disease knowledge management,general life management and treatment compliance in CSMS scale and total scores of the two groups after management were higher than those before management(P<0.05),and the scores of all dimensions and total score in the observation group after management were higher than those in the control group(P<0.05).The scores of Self-Rating Anxiety Scale(SAS),Self-Rating Depression Scale(SDS)and Minnesota Heart Failure Quality of life questionnaire(LiHFe)of the two groups after management were lower than those before management,and the scores of SAS,SDS and LiHFe in the observation group after management were lower than those in the control group(P<0.01).The 6-minute walking distance of the two groups after management were significantly longer than those before management(P<0.01),of which the observation group was longer than that of the control group(P<0.05).Conclusion:Application of stepped health education combined with integrated management for patients with coronary heart disease complicated with heart failure can significantly improve their self-management
