摘  要:目的:探讨1.5T MR磁敏感加权成像(SWI)在高压脑微出血(CMBs)检测中的应用价值。方法:选取在医院就诊的212例高血压患者纳入高血压组,按照匹配原则随机抽取87名在医院进行健康体检的受试者作为健康对照组,两组均行SWI和传统的T_(2)加权梯度回波序列(GRE)检查,观察不同受试者和序列检出率差异。结果:高血压组CMBs检出率显著高于健康对照组,差异有统计学意义(χ^(2)=52.726,P<0.05)。中度高血压CMBs检出率高于轻度高血压患者(χ^(2)=4.251,P<0.05),重度高血压CMBs检出率高于中度和轻度高血压患者(χ^(2)=7.516,P<0.05)。在轻度、中度和重度不同分级高血压患者中,SWI对CMBs的检出率均高于GRE,差异均有统计学意义(χ^(2)=5.462,χ^(2)=5.600,χ^(2)=4.292;P<0.05)。在患者皮层-皮层下、丘脑-基底节和小脑-脑干不同部位SWI病灶检出数量均高于GRE序列(χ^(2)=97.838,χ^(2)=112.299,χ^(2)=130.667;P<0.05)。结论:SWI对高血压患者CMBs的检出率和检出数量均高于GRE,不同研究者之间一致性高于GRE,可作为高血压患者CMBs检测的常规序列应用。Objective:To explore the application value of 1.5T MR susceptibility weighted imaging(SWI)in the detection of hypertensive cerebral microbleeds(CMBs).Methods:212 patients with hypertension in our hospital were selected and they were divided into hypertension group,and 87 healthy subjects who underwent physical examination in hospital were randomly selected as the control group according to the matching principle.SWI and traditional magnetic resonance gradient echo T_(2) imaging(GRE)were performed in both groups.The differences of detection rate of different subjects and sequences were observed.Results:The detection rate of CMBs in hypertension group was significantly higher than that in control group(χ^(2)=52.726,P<0.05).The detection rate of CMBS in patients with moderate hypertension was significantly higher than that in patients with mild hypertension(χ^(2)=4.251,P<0.05),and the detection rate of CMBS in patients with severe hypertension was significantly higher than that in patients with moderate and patients with mild hypertension(χ^(2)=7.516,P<0.05),respectively.In patients with different grades of hypertension,the detection rate of CMBs of SWI was significantly higher than that of GRE in mild,moderate and severe hypertension,and the differences of these between them were statistically significant(χ^(2)=5.462,χ^(2)=5.600,χ^(2)=4.292,P<0.05).The detected numbers of CMBs lesions below cortex-cortex,at thalamus-basal ganalia and epencephalon-brainstem of SWI were significantly higher than those of GRE sequence(χ^(2)=97.838,χ^(2)=112.299,χ^(2)=130.667,P<0.05),respectively.Conclusion:The detection rate and detected number of SWI for CMBs of patients with hypertension is higher than those of GRE,and the consistency among different researchers by using SWI is higher than that by using GRE,which can be used as a routine sequence to be promoted and applied in detecting CMBs of patients with hypertension.
