微创SuperPath入路半髋置换治疗老年股骨颈骨折的效果

摘　　要：目的探讨老年股骨颈骨折微创SuperPath入路半髋置换治疗效果。方法回顾性选取2020年9月至2022年6月广东省兴宁市第三人民医院骨科老年股骨颈骨折患者50例,依据半髋置换手术入路方式分为微创SuperPath入路组(观察组)和直接后外侧入路组(对照组),每组各25例。统计分析两组疼痛程度、起立-行走计时测试(TUG)、血红蛋白水平、关节活动度、髋关节功能、术后并发症发生情况、围手术期指标、治疗满意度。结果术后1 d、2周,观察组患者的疼痛视觉模拟评分(VAS)、TUG均低于对照组,差异有统计学意义(P<0.05)。观察组患者术后血红蛋白水平、外展角、前倾角、Harris评分均高于对照组,差异有统计学意义(P<0.05)。观察组患者的术后并发症总发生率为4.00%,低于对照组的32.00%,差异有统计学意义(P<0.05)。观察组患者的切口长度、开始负重时间短于对照组,术中出血量、术后引流量少于对照组,手术时间长于对照组,差异有统计学意义(P<0.05)。观察组患者的治疗满意度为96.00%,高于对照组的68.00%,差异有统计学意义(P<0.05)。结论老年股骨颈骨折微创SuperPath入路半髋置换治疗效果较直接后外侧入路半髋置换治疗效果好。Objective To explore the therapeutic effect of hip hemiarthroplasty through minimally invasive SuperPath approach for femoral neck fracture in elderly patients.Methods A total of 50 elderly patients with femoral neck fractures in the Department of Orthopedics,the Third People’s Hospital of Xingning City of Guangdong Province,from September 2020 to June 2022 were retrospectively selected and divided into the minimally invasive SuperPath approach group(the observation group)and the direct posterolateral approach group(the control group)according to the hip hemiarthroplasty approach,with 25 patients in each group.The pain severity,timed up-and-go test(TUG),hemoglobin level,joint range of motion,hip joint function,incidence of postoperative complications,perioperative indicators and treatment satisfaction were statistically analyzed in the two groups.Results 1 d and 2 weeks after operation,the visual analogue scale(VAS)and TUG scores of the observation group were lower than those of the control group,with statistically significant differences(P<0.05).The hemoglobin level,abduction angle,anteversion angle and Harris score of the observation group after operation were higher than those of the control group,with statistically significant differences(P<0.05).The incidence of postoperative complications in the observation group(4.00%)was lower than that in the control group(32.00%),with statistically significant difference(P<0.05).The incision length of patients and the time until weight-bearing in the observation group were shorter than those of patients in the control group;the intraoperative blood loss and postoperative drainage volume were less than those of the control group;the operation time was longer than that of the control group,with statistically significant differences(P<0.05).The treatment satisfaction of patients in the observation group(96.00%)was higher than that in the control group(68.00%),with statistically significant difference(P<0.05).Conclusion The therapeutic effect of hip hemiarthroplasty throu
