2型糖尿病合并高血压患者血脂、血糖代谢情况及其与高血压病程的关系分析

摘　　要：目的 探究2型糖尿病并高血压患者的血压病程与血脂、血糖代谢情况间的关系。方法 选取本院2020年1~12月收治的114例2型糖尿病患者为研究对象,将其中未合并高血压者纳入对照组(单一糖尿病患者,42例);将合并高血压者中的糖尿病先发者纳入A组(35例),将高血压先发者纳入B组(37例);测量并对比三组患者的血脂、血糖代谢情况,并分析其与高血压病程间的关系。结果 A/B组患者2hPBG、空腹胰岛素、GhbAl及HOMA指数水平均高于对照组(P<0.05);且B组患者上述指标高于A组(P<0.05)。同时,A/B组患者TC水平均高于对照组患者(P<0.05),且各组患者TG、HDL、LDL水平比较无差异(P>0.05)。经Pearson相关性分析可见,高血压病程与TC、2hPBG、GhbAl及HOMA指数有正相关关联(P<0.05)。结论 高血压病程与2型糖尿病并高血压患者的血脂及血糖代谢水平呈现正相关关系,故临床上应充分重视高血压疾病对患者血糖及血脂水平的影响,并将其作为2型糖尿病并高血压病症评估及预后参考指标,指导糖尿病并高血压患者的临床治疗。Objective To explore relationship between hypertension course and blood pressure and blood lipid,blood glucose metabolism of type 2 diabetes patients with hypertension.Methods The paper chose 114 cases type 2 diabetes patients in our hospital as study subjects(admission time:January to December 2020),those without hypertension were included in control group(42 cases with single diabetes);among hypertension patients,those with diabetes were included in group A(35 cases),those with hypertension were included in group B(37 cases).Blood lipid and blood glucose metabolism between three groups was measured and compared,relationship between them and hypertension course was analyzed.Results Levels of PBG,fasting insulin,GhbAl and HOMA index in group A/B were higher than control group(P<0.05);above indexes in group B were higher than group A(P<0.05).At the same time,TC level of patients in A/B group was higher than control group(P<0.05),there was no difference in TG,HDL and LDL levels among patients in each group(P>0.05).Pearson correlation analysis showed hypertension course was positively correlated with TC,2hPBG,GhbAl and HOMA index(P<0.05).Conclusion Hypertension course is positively related to level of blood lipid and blood glucose metabolism of patients with type 2 diabetes and hypertension,it is necessary to pay full attention to effect of hypertension on blood glucose and blood lipid levels of patients with type 2 diabetes and hypertension,which can be reference index for disease evaluation and prognosis of type 2 diabetes and hypertension to guide their clinical treatment.
